Pharmaceutical action:
 An antiviral agent derived from adamantane; effective against various strains of influenza A virus, Herpes simplex viruses of type I and II, tick-borne encephalitis viruses (Central European and Russian spring-summer arboviruses of the family. Flaviviridae). It has antitoxic and immunomodulatory effects.
The polymer structure ensures long-term circulation of rimantadine in the body, which allows it to be used not only for therapeutic, but also for preventive purposes. Suppresses the early stage of specific reproduction (after the virus enters the cell and before the initial RNA transcription); induces the production of interferons alpha and gamma, increases the functional activity of natural killer lymphocytes (NK cells), T-and B-lymphocytes.
Being a weak base, it increases the pH of endosomes that have a membrane of vacuoles and surround viral particles after they enter the cell. Preventing acidification in these vacuoles blocks the fusion of the viral envelope with the endosome membrane, thus preventing the transfer of viral genetic material to the cell cytoplasm. Rimantadine also inhibits the release of viral particles from the cell, i. e. interrupts the transcription of the viral genome.
Prophylactic use of rimantadine in a daily dose of 200 mg reduces the risk of flu, as well as reduces the severity of flu symptoms and serological response. Some therapeutic effect may also occur when it is prescribed in the first 18 hours after the first flu symptoms develop.

From the gastrointestinal tract: Â dry mouth, nausea, vomiting, loss of appetite, epigastric pain, flatulence;
From the central nervous system: Â headache, dizziness, insomnia, neurological reactions, impaired concentration, drowsiness, anxiety, increased excitability, fatigue;
Other services: Â hyperbilirubinemia, allergic reactions (skin rash, pruritus, urticaria).
Remantadine reduces the effectiveness of antiepileptic drugs. Paracetamol and ascorbic acid reduce the maximum concentration of rimantadine in blood plasma by 11%.
Cimetidine reduces the clearance of Remantadine by 18%.
Adsorbents, binders and enveloping agents (acetazolamide, sodium bicarbonate, etc. ) enhance the effectiveness of Remantadine due to a decrease in its excretion by the kidneys.
Urine acidifying agents (for example, ascorbic acid) reduce the effectiveness of Remantadine® due to increased renal excretion.

Flu treatment: Â adults – on day 1 – 300 mg/day in 1-3 doses; on days 2 and 3-100 mg 2 times a day; on days 4 and 5-100 mg 1 time a day; children 7-10 years-50 mg 2 times a day,11-14 years-50 mg 3 times a day, for 5 days.
Flu prevention: Â adults – 50 mg once a day, for 30 days.
Prevention of tick-borne encephalitis: Â no later than 48 hours after a tick bite-100 mg 2 times a day, with an interval of 12 hours, for 3-5 days; risk groups (persons living or located in a forest area) – 100 mg 2 times a day for 15 days.
Treatment of acute herpes infection: Â 100 mg / day for 1 dose for 3 days; prevention of exacerbations-100 mg 1 time in 3 days.
Patients with a history of epilepsy and anticonvulsant therapy are prescribed a maximum daily dose of 100 mg / day (with simultaneous appointment of anticonvulsant therapy due to the risk of developing an epileptic seizure). In the elderly and in CRF, the dose should be reduced.
Symptoms: Â agitation, hallucinations, arrhythmia.
Treatment: Â gastric lavage, symptomatic therapy: measures to maintain vital functions. Rimantadine is partially eliminated by hemodialysis.
When using Remantadine, it is possible to exacerbate chronic concomitant diseases. Elderly patients with arterial hypertension have an increased risk of hemorrhagic stroke.
If there is a history of epilepsy and ongoing anticonvulsant therapy with Remantadine, the risk of developing an epileptic seizure increases. In such cases, Remantadine is used at a dose of up to 100 mg per day simultaneously with anticonvulsant therapy.
In influenza caused by the B virus, Remantadine has an antitoxic effect. Preventive treatment is effective in contact with sick people, when the infection spreads in closed groups, and when there is a high risk of developing the disease during an influenza epidemic.
Viruses that are resistant to the drug may appear. Remantadine does not affect the ability to drive a vehicle, but individuals who experience dizziness, headache, or other central nervous system side effects should exercise caution. Use with caution in case of lactase deficiency, lactose intolerance, lactose/isomaltose malabsorption syndrome, because the drug contains lactose.
