Composition
Active ingredient:
rimantadine hydrochloride – 50 mg in 1 tablet.
Excipients of Remantadine tablets:
potato starch,
magnesium stearate,
microcrystalline cellulose,
lactose monohydrate.
Pharmacological action
Rimantadine is active against influenza virus type A (especially type A2). The substance is a weak base, so its action is due to an increase in the pH of endosomes, which include the vacuole membrane. These endosomes surround viral particles after they enter the cell. Acidification is prevented in these vacuoles, which makes it possible to block the fusion of the viral envelope with the endosome membrane. This prevents the transmission of viral genetic material to the cytoplasm of the cell. Rimantadine also suppresses output
Indications
Prevention and early treatment of influenza A in adults and children over 7 years of age.
Recommendations for use
Remantadine should be taken orally after a meal: swallow the tablets / capsules whole and drink enough water. Recommended doses:
- Adults and adolescents over 14 years of age: on the 1st day-100 mg (1 capsule or 2 tablets) 3 times a day, on the 2nd-3rd days-100 mg 2 times a day, on the 4th-5th days-100 mg 1 time a day. On the first day of the disease, a single dose of Remantadine 300 mg (3 capsules or 6 tablets) is allowed;
- Children 11-14 years: 50 mg (1 tablet) 3 times a day;Â
- Children 7-10 years: 50 mg 2 times a day.
The duration of treatment is 5 days. It is recommended to start taking the drug immediately after the first symptoms of flu appear. Remantadine is most effective if you start treatment within the first 24-48 hours. Elderly people, patients with hepatic insufficiency and chronic renal failure are prescribed 100 mg 1 time a day. To prevent influenza, adults and children over 7 years of age are prescribed 1 tablet of Remantadine 1 time a day for 10-15 days.

Side effects
Remantadine is generally well tolerated. In rare cases, the following side effects are noted: :
- From the central nervous system: neurological reactions, insomnia, headache, impaired concentration, dizziness, anxiety, drowsiness, fatigue, increased excitability;
- Gastrointestinal disorders: loss of appetite, anorexia, dry mouth, flatulence, epigastric pain, nausea, vomiting;Â
- Allergic reactions: pruritus, rash, urticaria;Â
- Other: hyperbilirubinemia.
Interaction
Cimetidine reduces the clearance of rimantadine by 18%, paracetamol reduces its maximum concentration in blood plasma by 11%. Urine acidifying agents (for example, ascorbic acid and ammonium chloride) accelerate the excretion of rimantadine by the kidneys, which reduces its effectiveness. Urinary alkalizing agents (for example, sodium bicarbonate and acetazolamide), on the contrary, reduce the excretion of rimantadine by the kidneys, which increases its effect. Astringent, adsorbent and enveloping agents reduce the absorption of rimantadine. Remantadine reduces the effectiveness of concomitantly administered antiepileptic drugs.
Overdose
The main symptoms of Remantadine overdose are arrhythmia, hallucinations, and nervous excitement. In this case, gastric lavage is performed and symptomatic therapy is prescribed. It is also recommended to resort to measures aimed at maintaining vital functions. Rimantadine is partially eliminated by hemodialysis.
Special instructions
During Remantadine treatment, there is a possibility of exacerbation of concomitant chronic diseases. Elderly people with arterial hypertension have an increased risk of developing a hemorrhagic stroke, and patients with a history of epilepsy or who are receiving anticonvulsant therapy at the same time have an increased risk of developing an epileptic seizure. In the latter case, the daily dose of rimantadine should not exceed 100 mg. Preventive use of the drug is effective if there is a high risk of developing the disease during an epidemic of influenza, contact with sick people or the spread of infection in closed groups. In influenza caused by the B virus, rimantadine has an antitoxic effect. Viruses that are resistant to the drug may appear. Due to the possibility of developing side effects from the central nervous system during treatment, it is recommended to use caution when driving vehicles and performing potentially dangerous types of work that require increased attention, high speed of mental and/or physical reactions.
