Composition
Active ingredients:
in 1 dose – specific immunoglobulins of the protein fraction of the blood serum of horses
hyperimmunized with the venom of the common viper 150 AE.
Pharmacological action
The drug is an immunoglobulin fraction of the blood serum of horses immunized with viper venom, purified and concentrated by peptic digestion and salt fractionation, containing specific antibodies.
The antibodies contained in the serum neutralize the venom of the common viper.
Indications
Treatment of people bitten by a viper.
Contraindications
Contraindication to continuing the use of serum is only the development of anaphylactic shock with the introduction of 0.1-0.25 ml of serum.
Side effects
The introduction of serum against viper venom can be accompanied by the development of various allergic reactions: immediate, occurring immediately after use or after a few hours and manifested by symptoms of anaphylactic shock, early – on the 2nd day after use, and long-term – on 5-10 days.
The latter are accompanied by a symptom complex of serum sickness (fever, itching and skin rashes, joint pain, etc. ).
If an anaphylactic reaction occurs, the use of serum is suspended. Subcutaneously administered epinephrine (epinephrine) (0.3-1 ml), cordiamine (1.5-2 ml), prednisone 25 ml or hydrocortisone (50-100 ml), strophanthin (0.5 ml 0.04%) with glucose (20 ml 40% solution). Taking into account the possibility of shock during the use of serum, each vaccinated person should be provided with medical supervision within 1 hour after the end of the drug use.
The introduction of serum should be registered in the established accounting forms with mandatory indication of the date, method and time of use, patient’s reaction, manufacturer and batch number of the drug.
Interaction
Compatibility with other medicinal products containing a foreign protein must be taken into account.
How to take, course of use and dosage
Serum is administered subcutaneously, intramuscularly or intravenously as early as possible after a snake bite.
Providing first aid to the bitten person on the spot
The victim is placed in the shade and given a plentiful drink: tea, coffee, milk, broth, water (the use of alcoholic beverages is prohibited) and 1 therapeutic dose of serum is administered, regardless of the body weight of the bitten person. To avoid anaphylactic shock or other allergic complications, before the introduction of serum, the victim is given 1-2 tablets of any antihistamine (diphenhydramine, Pipolphen, tavegil, etc. ) inside. The serum is injected subcutaneously fractional (occasionally) in any part of the victim’s body: first 0.1 ml, in the absence of a reaction,0.25 ml is administered after 10-15 minutes, and then after 15 minutes, in the absence of adverse reactions, the entire remaining serum. The opened ampoule should be covered with a sterile bandage or cotton swab.
After providing first aid, it is necessary to ensure emergency hospitalization of the victim in the nearest medical facility, where treatment will continue. Transport the patient in a supine position. The bitten leg is bandaged to the healthy one, giving it a slightly elevated position. When bitten in the arm, it is fixed in a bent position with a cloth thrown over the neck.
Assistance in a medical facility
The total administered dose of serum is determined by the doctor depending on the degree of intoxication: for mild intramuscular 1-2 doses, for severe 4-5 doses, taking into account the dose administered before hospitalization.
In case of particularly severe intoxication, the serum is recommended to be administered intravenously slowly by drip after dilution (1/5-1/10) with a sterile 0.9% sodium chloride solution heated to a temperature of (37±1)°C for injection. Rate of use: first 1 ml for 5 minutes, then 1 ml per minute. In exceptional cases, if it is impossible to perform a drip infusion, slow jet use of the therapeutic dose of serum without dilution with a syringe is allowed. In order to avoid possible allergic reactions,60-90 mg of prednisone is injected into the patient in a jet before the intravenous infusion of serum. Intravenous use of serum is only permitted for medical personnel!
Children use the same dosage principle as adults, regardless of weight and age.
In some cases, if the bite site is contaminated with earth, there are wounds, or any type of tissue necrosis, it is necessary to carry out emergency tetanus prevention measures in accordance with the instructions for tetanus prevention.

Ampoules with serum are carefully examined before opening. The drug should not be used when:
- changes in the physical properties (cloudy contents of the ampoule, the presence of undeveloped flakes);
- any damage to the integrity of the ampoules (cracks, etc. ).
A first-aid drug that was stored in the conditions of expeditions with a violation of the temperature regime is not suitable for use at the end of the expedition.
