Composition
1 ml of infusion solution contains:
Active ingredient:
human albumin 100 mg;
Auxiliary substances:
sodium caprylic acid,
sodium chloride,
water for injection.
Pharmacological action
Pharmaceutical group:
plasma-substituting agent.
Pharmaceutical action:
A plasma-substituting agent obtained by fractionation of human plasma.
Replenishes the deficiency of plasma albumin, maintains colloidal osmotic (oncotic) blood pressure, quickly increases blood pressure and BCC, increases the transition and promotes retention of tissue fluid in the bloodstream, increases the reserves of protein nutrition of tissues and organs.
Indications
The decrease in the concentration of albumin in plasma is less than 30 g/l or colloid-oncotic pressure less than 15 mm Hg. St., or a reduction in total protein less than 50 g/l: hypoalbuminemia different Genesis – shock (hemorrhagic, traumatic, thermal), acute blood loss (reduction of BCC more than 25-30%), purulent-septic conditions, liver disease violation albuminiferous function, kidney disease (nephritis, nephrotic syndrome), burn disease;
surgery with use of cardiopulmonary bypass;
therapeutic plasmapheresis;
hemolytic disease of the newborn during exchange transfusion;
conducting preoperative hemodilution and procurement of autologous blood components;
swelling of the brain (hyperoncotic solutions);
gastrointestinal diseases with malabsorption or terrain.
Use during pregnancy and lactation
During pregnancy, the use of human albumin is possible only in cases of extreme necessity.
Contraindications
- Hypersensitivity.
- Severe forms of heart failure (due to the possible occurrence of acute heart failure).
- Thrombosis.
- Severe form of arterial hypertension.
- Ongoing internal bleeding.
- Hypervolemia.
- Pulmonary edema.
- Severe anemia.
- Brain edema.
With caution prescribed for: renal failure, bronchial asthma, allergic rhinitis, angioedema.
Side effects
use of an albumin solution of 10%, as a rule, is not accompanied by side reactions and complications, sometimes the development of:
- Allergic reactions of varying severity (urticaria and others).
- Hyperthermia.
- Pain in the lumbar region.
Interaction
Concomitant use of albumin with ACE inhibitors increases the risk of hypotension.
How to take, course of use and dosage
A 10% albumin solution is administered intravenously by drip or jet. A single dose of the drug depends on the concentration of albumin solution, initial condition and age of the patient. Albumin solutions of 10% are administered in a dose of 200-300 ml.
The rate of use of 10% albumin solution is not more than 40 drops per minute.
Jet use of albumin solutions is acceptable for shocks of various origins to rapidly increase blood pressure.
In pediatric practice, the dosage of albumin solutions is calculated taking into account the concentration in milliliters per kilogram of body weight (no more than 3 ml/kg of body weight of the child).
Overdose
In case of overdose or too rapid use of the drug, hypervolemia may occur (development of pulmonary edema, increased arterial and central venous pressure).
At the initial signs of an anaphylactic reaction or signs of overdose (headache, shortness of breath, suffocation), albumin transfusion should be stopped immediately and, without removing the needle from the vein, appropriate drugs should be administered (glucocorticosteroids, vasopressors, if indicated).
Special instructions
Before using the drug, the doctor registers the label data (name of the drug, manufacturer, batch number) in the patient’s medical history.
In the elderly, rapid use of a 10% albumin solution should be avoided, as this can lead to an overload of the cardiovascular system.
Before starting the infusion, albumin solutions should be warmed to room temperature. The drug is not suitable for use in containers with broken integrity or labeling, with changes in physical properties (color change, turbidity of the solution, the presence of suspension, sediment), with an expired shelf life, improper storage.
