Active ingredient:
Proxymetacaine hydrochloride 5.0 mg;
Auxiliary substances:
benzalkonium chloride, solution equivalent to benzalkonium chloride,
glycerol,
concentrated hydrochloric acid and / or sodium hydroxide for pH adjustment,
purified water.
Pharmacological action
Proxymetacaine is a local anesthetic used in ophthalmology. The effect of Alcaine usually develops 20-30 seconds after instillation and lasts for 15 minutes or longer.
Proxymetacaine inhibits the permeability of cell membranes to sodium ions, which prevents the generation of impulses in nerve endings and the conduction of impulses through nerve fibers.
Pharmacokinetics
When applied topically, systemic absorption is low.
Indications
Local anesthesia in ophthalmology for short-term diagnostic and therapeutic procedures that require rapid and short-term anesthesia:
- cataract surgery, removal of suture thread from the cornea or conjunctiva;
- tonometry, gonioscopy;
- removal of foreign bodies from the cornea;
- conjunctival scraping for diagnostic purposes.
Use during pregnancy and lactation
There is no sufficient experience with the use of the drug Alcaine during pregnancy, breast-feeding and in children.
It is possible to use the drug in children, during pregnancy and lactation as prescribed by the attending physician, if the expected therapeutic effect exceeds the risk of possible side effects.
Contraindications
Individual hypersensitivity to the components of the drug Alcaine.
Use with caution: Use with caution in patients with allergic reactions, heart disease, and hypothyroidism.
Side effects
As a rule, it is well tolerated.
Itching, burning, and hyperemia of the conjunctiva may occur; lacrimation or rapid blinking.
Rarely and usually immediately after instillation, severe allergic reactions may occur, such as epithelial keratitis; corneal erosion, corneal filamentous infiltrates, iritis.
With prolonged use, slower wound healing is possible.
It should be borne in mind the possibility of systemic toxicity with prolonged use (which is manifested by stimulation of the central nervous system with its subsequent suppression), although when using the drug topically as an anesthetic, such a reaction to it is extremely rare.
How to take, course of use and dosage
Locally.
- When performing tonometry and other procedures of short duration
,1-2 drops are instilled immediately before the procedure.
- For minor surgical procedures (removal of a foreign body or suture thread)
Instill 1-2 drops every 5-10 minutes (1-3 doses).
- If long-term anesthesia is required (for cataract removal)
Instill 1-2 drops every 5-10 minutes (3-5 doses). After surgical manipulation, it is necessary to cover the eye with a bandage, because the “blinking” reflex is temporarily eliminated.
Special instructions
It is important to protect the eye from irritating chemicals and foreign bodies during the entire period of anesthesia, as well as not to rub the eye.
Patients should be warned not to touch their eyes until the anesthetic has passed.
The bottle must be closed after each use. Do not touch the tip of the pipette to the eye.
