Hypersensitivity to co-trimoxazole, trimethoprim, sulfonamides or any component of the drug, pregnancy, breast-feeding, children under 6 years of age (for this dosage form), established diagnosis of liver parenchyma damage, severe renal failure if it is not possible to determine the concentration of the drug in blood plasma (not recommended for use with creatinine clearance below 15 ml/min), severe hematological diseases (aplastic anemia, B12-deficiency anemia, agranulocytosis, leukopenia, megaloblastic anemia, hyperbilirubinemia in children associated with folic acid deficiency), glucose-6-phosphate dehydrogenase deficiency (the likelihood of developing hemolysis); simultaneous use with dofetilide, paclitaxel and amiodarone; simultaneous use with clozapine should be avoided, since the ability of the latter to cause aganulocytosis is known.

Co-trimoxazole is a broad-spectrum antibacterial agent.
It disrupts folate synthesis (prevents PABA acceptance), inhibits dihydrofolate reductase and folinic acid formation, and reduces the synthesis of purines, pyrimidines, amino acids, and protein in microbial cells.

From the digestive system: nausea, vomiting, diarrhea, glossitis, diarrhea, abdominal pain, lack of appetite, pseudodiphtheria inflammation of the intestines, increased liver enzymes and serum creatinine, inflammation of the oral cavity, inflammation of the tongue, inflammation of the pancreas, cholestatic hepatitis. Allergic reactions: allergic myocarditis, chills, drug-induced fever, photosensitization, anaphylactic symptoms, skin rash, angioedema, Stevens-Jones syndrome, Lyell’s syndrome, Hematopoietic disorders: leukopenia, neutropenia, thrombocytopenia, agranulocytosis, megaloblastic anemia. Urinary system disorders: crystalluria, hematuria, interstitial nephritis. Metabolism: hypokalemia, hyponatremia. Interaction with other medicinesw Ith simultaneous use of co-trimoxazole with indirect anticoagulants, the effect of the latter is significantly enhanced due to a slowdown in the inactivation of the latter, as well as their release from binding to plasma proteins. When used in combination with some sulfonylureas, the hypoglycemic effect may be enhanced. Concomitant use of co-trimoxazole and methotrexate may lead to increased toxicity of the latter (in particular, to the appearance of pancytopenia). Under the influence of butadione, Indometacin, naproxen, salicylates and some other NSAIDs, the effect of co-trimoxazole may increase with the development of undesirable effects, since active substances are released from binding to blood proteins and their concentration increases. Concomitant use of diuretics and co-trimoxazole increases the likelihood of thrombocytopenia caused by the latter, especially in elderly patients. In the case of simultaneous use of chloridine with co-trimoxazole, the antimicrobial effect is enhanced, since chloridine inhibits the formation of tetrahydrofolic acid, which is necessary for the synthesis of nucleic acids and proteins. In turn, sulfonamides inhibit the formation of dihydrofolic acid, which is a precursor of tetrahydrofolic acid. This combination is widely used in the treatment of toxoplasmosis. The absorption of co-trimoxazole when co-administered with cholestyramine decreases as a result of the formation of insoluble complexes, which leads to a decrease in their concentration in the blood.
The dosage regimen is individual, depending on the severity of the disease, the sensitivity and type of pathogen, and the age of the patient. Children from 6 to 12 years: 1 tablet (480 mg) 2 times a day. Adults and children over 12 years of age – 960 mg once or 480 mg 2 times a day. For severe infections-480 mg 3 times a day, for chronic infections, the maintenance dose is 480 mg 2 times a day. For uncomplicated infections, prescribe: over 12 years of age and adults,1 tablet 2 times a day. The duration of the course of treatment is from 5 to 14 days. In severe and/or chronic infectious diseases, it is permissible to increase the single dose by 30-50%. The drug is taken during or after a meal. It is recommended to drink an alkaline drink (milk, mineral water).
Symptoms: anorexia, nausea, weakness, abdominal pain, headache, drowsiness, hematuria, crystalluria. Treatment: heavy drinking, gastric lavage, correction of electrolyte disorders. If necessary, hemodialysis is prescribed. Chronic overdose is characterized by bone marrow depression (pancytopenia). Treatment and prevention: use of folic acid (5-15 mg daily).
During therapy, you should consume a sufficient amount of fluids. The drug is prescribed with caution to patients with possible folic acid deficiency, a history of allergic reactions, bronchial asthma, impaired liver, kidney, and thyroid function. With prolonged use of the drug, it is necessary to systematically conduct studies of the picture of peripheral blood, the functional state of the liver and kidneys. Additional folic acid supplementation is recommended for elderly patients. In case of impaired renal function, the dose should be reduced, and the intervals between doses should be increased. It is not recommended to take the drug for young children.
