Composition

of 100 g of ointment contains the Active ingredient: diclofenac sodium-2.0 g;

excipients: karbomer (arespol, polyacrylate radosity) – 1.0 g, sodium hydroxide 0.37 g, Benzyl benzoate (Benzyl benzoate medical) – 6,0 g, Polysorbate-80 (tween 80) – 1.0 g, methyl parahydroxybenzoate (nipagin) – 0,08 g, propylpyrimidine-benzoate (nipazol) – 0.02 g, purified water – up to 100,0 G.

Pharmacological action

NSAIDs, a derivative of phenylacetic acid, anti-inflammatory and analgesic effect. Indiscriminately inhibiting COX 1 and COX 2, it disrupts the metabolism of arachidonic acid, reduces the amount of Pg, including in the focus of inflammation.

Pharmacokinetics

When applied, the systemic absorption is not more than 6%. Protein binding is 99.7%. It is excreted by the kidneys. When applied to the affected joint area, the concentration in synovial fluid is higher than in plasma.

Indications

Inflammatory diseases of the musculoskeletal system (rheumatoid arthritis, psoriatic, juvenile chronic arthritis, ankylosing spondylitis, rheumatic soft tissue lesions, gouty arthritis); traumatic inflammation of tendons, ligaments, muscles and joints (as a result of sprains, stress and bruises).

Pain syndrome (lumbago, sciatica, neuralgia, myalgia, tendovaginitis, bursitis, rheumatic soft tissue lesions; degenerative diseases of the musculoskeletal system: deforming osteoarthritis, osteochondrosis).

Use during pregnancy and lactation

Due to the lack of data on the use of the drug in pregnant women, the use of the drug during the first and second trimesters of pregnancy is recommended only as prescribed by a doctor, comparing the benefit to the mother and the risk to the fetus.

The drug is not recommended for use during breastfeeding. If the use of the drug is still necessary, then it should not be applied to the mammary glands or on a large surface of the skin and should not be used for a long time.

Contraindications

Hypersensitivity (including to other NSAIDs), younger children (up to 6 years), violation of the integrity of the skin at the site of the intended application.

With caution

Hepatic porphyria (exacerbation), erosive and ulcerative lesions of the gastrointestinal tract (in the acute phase), severe liver and kidney function disorders, CHF; bronchial asthma, urticaria or acute rhinitis caused by taking ASA or other NSAIDs; blood clotting disorders (including hemophilia, prolongation of bleeding time, tendency to bleed), old age, pregnancy (third trimester), breastfeeding.

Side effects

From the skin: eczema, photosensitization, contact dermatitis (itching, hyperemia, swelling of the treated skin area, papular-vesicular rashes, peeling).

Allergic reactions (itching and burning of the skin, erythematous skin rash, urticaria, angioedema, systemic anaphylactic reactions (including shock).

Interaction

Not described.

How to take, course of use and dosage

Externally. Ointment – 2-4 g (1 g corresponds to about 2 cm of the squeezed out strip of ointment) is applied in a thin layer, lightly rubbing into the skin, over the focus of inflammation 2-3 times a day; after application, you need to wash your hands, unless this area is being treated. The maximum daily dose is 8 g.

The course of treatment is 7-14 days.

The drug has an optimal combination of analgesic and anti-inflammatory effects and is well tolerated. Therefore, in the absence of contraindications, it can be used even for a long time.

The duration of treatment depends on the indications and the observed effect (to enhance the effect, the drug can be used together with other dosage forms of diclofenac in consultation with the doctor).

Overdose

Symptoms: due to the low systemic absorption when applying the ointment, overdose is unlikely. If accidentally ingested, systemic adverse reactions may occur.

Treatment: gastric lavage, induction of vomiting, activated charcoal, forced diuresis, symptomatic therapy. Dialysis is not effective due to the high degree of binding to diclofenac proteins (about 99%).

Description

The ointment is white or white with a grayish tinge of color.

Special instructions

Apply only to undamaged areas of the skin. After applying, do not apply an occlusive dressing.

With prolonged use and/or application on large surfaces, systemic side effects may develop due to resorptive action.

Avoid contact with eyes, mucous membranes, or open wounds.

Apply only externally.

The drug does not affect the ability to drive vehicles and manage mechanisms.

Form of production

Ointment for external use 2% in aluminum tubes of 30.0 g. Each tube together with the instructions for use is placed in a cardboard pack.

Storage conditions

Store in a dry place protected from light at a temperature of 8 to 15 °C. Keep out of reach of children.

Shelf

life is 2 years. Do not use after the expiration date.
