Composition

levocarnitine (carnifit) – 100 mg;

water for injection up to 1 ml

Pharmacological action

L-Carnitine increases the resistance of nervous tissue to damaging factors (hypoxia, trauma, intoxication, etc. ), inhibits the formation of keto acids and anaerobic glycolysis, reduces the degree of lactic acidosis.

L-Carnitine (a natural substance related to B vitamins) is involved in metabolic processes as a carrier of fatty acids across cell membranes from the cytoplasm to the mitochondria, where these acids are oxidized (beta-oxidation process) to form a large amount of metabolic energy (in the form of ATP).

The drug replenishes the alkaline reserve of blood, helps restore autoregulation of cerebral hemodynamics and increase blood supply to the affected area, accelerates reparative processes in the lesion and has an anabolic effect.

Indications

— in the complex therapy in acute hypoxic conditions (acute brain hypoxia, ischemic stroke, transient ischemic attack);

acute, sub-acute and recovery periods of violations of cerebral circulation;

— encephalopathy and various traumatic and toxic brain damage, during the recovery period after surgery;

primary and secondary carnitine deficiency, including patients with chronic renal failure, hemodialysis;

— cardiomyopathy;

— ischemic heart disease (angina, acute myocardial infarction, postinfarction state);

— hypoperfusion due to cardiogenic shock and other disorders of metabolism in the myocardium.

Contraindications

-pregnancy;

– lactation period;

– hypersensitivity.

Side effects

Possible allergic reactions, muscle weakness (in patients with uremia).

With rapid use (80 drops / min or more), pain along the vein may occur, passing with a decrease in the rate of use.

Interaction

Glucocorticosteroids contribute to the accumulation of the drug in tissues (except for the liver), other anabolics enhance the effect.

How to take, course of use and dosage

Elkar is administered intravenously by slow drip or jet (2-3 min) or in/m. Before intravenous use, the contents of the ampoule are dissolved in 100-200 ml of solvent (0.9% sodium chloride solution or 5% dextrose (glucose) solution).

In acute disorders of cerebral circulation,1 g/day is prescribed for 3 days, and then 0.5 g/day for 7 days. After 10-12 days, repeated courses are possible within 3-5 days.

When prescribing the drug in the subacute and recovery period, with dyscirculatory encephalopathy and various brain lesions, carnitine deficiency, patients are given an elkar solution at the rate of 0.5-1 g/day intravenously (drip, jet) or in / m (2-3 times a day) without dilution for 3-7 days. If necessary, a second course is prescribed after 12-14 days.

Intravenous use, slowly (2-3 min. ) is prescribed for secondary carnitine deficiency during hemodialysis-2 g once (after the procedure); for acute myocardial infarction, acute heart failure-3-5 g/day, divided into 2-3 doses in the first 2-3 days, followed by a 2-fold dose reduction; for cardiogenic shock-3-5 g/day, divided into 2-3 doses until the patient comes out of shock. Then they switch to oral use of Elkar.

Overdose

Symptoms:  dyspeptic disorders, myasthenic disorders (in patients with uremia).

Treatment:  gastric lavage, taking activated charcoal.

Special instructions

Treatment of children under 3 years of age should be carried out under the supervision of a doctor.

Influence on the ability to drive motor vehicles and manage mechanisms:

the drug does not affect the performance of potentially dangerous activities that require special attention and quick reactions (driving a car and other vehicles, working with moving mechanisms, working as a dispatcher and operator, etc. ).
