Composition

furosemide 40 mg;
Auxiliary substances:
lactose;
potato starch;
MCC;
povidone;
gelatin;
magnesium stearate;
colloidal silicon dioxide;
talc.
Pharmacological action
of Furosemide-natriuretic, diuretic.
Inhibits the reabsorption of sodium ions in the ascending knee of the Henle loop, proximal and distal tubules.
Increases the excretion of bicarbonates, phosphates, calcium, magnesium.
Indications
- Edema in heart failure, diseases of the liver and kidneys, obstruction of the venous outflow, after burns and venous thrombosis;
- ascites (in the background malignant neoplasms, hepatic and cardiac origin);
- peripheral edema;
- pulmonary edema, including menacing, post-traumatic brain edema, arterial hypertension;
- toxemia of pregnancy, birth eclampsia, premenstrual tension, prepojena uremia, hyperkalemia, conducting forced diuresis (toxicity of chemical compounds, including barbiturates).
Contraindications
Hypersensitivity, anuria, uremia, hepatic coma, mechanical obstruction of the urinary tract, dyselectrolyteemia.
Side effects
From the cardiovascular system: Â reduced blood pressure, orthostatic hypotension, collapse, tachycardia, arrhythmias, decreased BCC.
From the central nervous system and peripheral nervous system: Â dizziness, headache, myasthenia gravis, calf muscle spasms (tetany), paresthesia, apathy, adynamia, weakness, lethargy, drowsiness, confusion.
From the side of the senses: Â visual and hearing impairments.
From the digestive system: Â decreased appetite, dry mouth, thirst, nausea, vomiting, constipation or diarrhea, cholestatic jaundice, pancreatitis (exacerbation).
From the genitourinary system: Â oliguria, acute urinary retention (in patients with prostatic hypertrophy), interstitial nephritis, hematuria, decreased potency.
From the hematopoietic system: Â leukopenia, thrombocytopenia, agranulocytosis, aplastic anemia.
From the side of water-electrolyte metabolism: Â hypovolemia, dehydration (risk of thrombosis and thromboembolism), hypokalemia, hyponatremia, hypochloremia, hypocalcemia, hypomagnesemia, metabolic alkalosis.
From the side of metabolism: Â hypovolemia, hypokalemia, hyponatremia, hypochloremia, hypokalemic metabolic alkalosis (as a result of these disorders – hypotension, dizziness, dry mouth, thirst, arrhythmia, muscle weakness, convulsions), hyperuricemia (with possible exacerbation of gout), hyperglycemia.
Allergic reactions: Â purpura, urticaria, exfoliative dermatitis, erythema multiforme, vasculitis, necrotizing angiitis, pruritus, chills, fever, photosensitization, anaphylactic shock.
Other services: Â when administered intravenously (additionally) – thrombophlebitis, renal calcification in premature infants.
Interaction
When Furosemide is co-administered with cardiac glycosides, the risk of developing glycoside intoxication increases, and when co – administered with corticosteroids, the risk of hypokalemia increases. When used concomitantly, Furosemide potentiates the effect of muscle relaxants. Concomitant use of Furosemide with aminoglycosides or cephalosporins and cisplatin may increase their concentration in blood plasma, which may lead to the development of nephrotoxic and ototoxic effects. Concomitant use of NSAIDs may reduce the diuretic effect of Furosemide. With the simultaneous use of Furosemide and antihypertensive drugs, the hypotensive effect is potentiated. When used concomitantly with Furosemide, the effect of hypoglycemic drugs and pressor amines may be weakened.
How to take, course of use and dosage
Inside; adults with edema — 40 mg once a day (up to 80-120 mg / day), after increasing diuresis, the dose is reduced to maintenance-40 mg every other day.
Children — at the rate of 1-2 mg / kg / day.
Overdose
Symptoms: Â decreased blood pressure, collapse, shock, hypovolemia, dehydration, hemoconcentration, arrhythmias, acute renal failure with anuria, thrombosis, thromboembolism, drowsiness, confusion, flaccid paralysis, apathy.
Treatment: Â correction of water-salt balance and acid-base state (CBS), replenishment of circulating blood volume, symptomatic treatment.
There is no specific antidote.
Special instructions
With caution, the drug is prescribed for severe cardiovascular insufficiency, with prolonged therapy with cardiac glycosides, as well as for elderly patients with severe atherosclerosis. Before starting treatment, you should compensate for serious violations of the water-electrolyte balance. Monitoring of laboratory parameters During treatment, the level of electrolytes, carbonates, and urea should be regularly monitored (especially when using doses higher than 80 mg). Influence on the ability to drive motor vehicles and manage mechanismthe question of the possibility of engaging in potentially dangerous activities that require increased attention and speed of psychomotor reactions should be resolved after evaluating the individual patient’s reaction, due to a possible decrease in the ability to concentrate.
Storage conditions
