Composition

Pharmacological action

Guttalax Express is a local laxative from the group of diphenylmethane derivatives with a slight antiresorptive effect. After hydrolysis in the large intestine, bisacodyl stimulates its peristalsis and promotes the accumulation of water and electrolytes in the lumen of the large intestine. This leads to stimulation of bowel movements, reduced evacuation time, and softened stools.

The action is caused by direct stimulation of nerve endings in the mucous membrane of the large intestine.

The time of development of the laxative effect of the drug is 20 minutes, in some cases – 45 minutes.

Bisacodyl, being a laxative that acts at the level of the colon, stimulates the natural evacuation process in the lower gastrointestinal tract. Therefore, bisacodyl has no effect on the digestion or absorption of high-calorie foods or essential nutrients in the small intestine.

Indications

• Constipation caused by hypotension of the colon (including chronic constipation, regulation of stool in the postpartum period, in bedridden patients);
• preparation for instrumental and X-ray examinations;
• preoperative preparation, postoperative period.

Recommendations for use

For constipation

Adults and children over 10 years of age – 1 suppository per day.

Do not exceed the maximum recommended daily dose (10 mg).

The suppository is inserted into the rectum with the pointed end facing forward.

The action takes place in about 20 minutes (from 10 minutes to 30 minutes).

Do not use the drug daily without consulting a doctor and determining the cause of constipation for more than 10 days.

When preparing for research, pre-and postoperative treatment

When preparing for research, pre – and postoperative treatment, and medical conditions that require easier defecation, the drug should be used under medical supervision. It is recommended in combination with tablets to achieve complete emptying. In addition to the 2-4 tablets taken the day before, one suppository is recommended in the morning.

The decision on the required number of tablets of the drug to be taken on the eve of the study/operation is made by the attending physician, based on the patient’s clinical condition and the volume of the upcoming manipulation.

Contraindications

• Hypersensitivity to the active ingredient or excipients;
• intestinal obstruction;
• obstructive bowel diseases;
• acute diseases of the abdominal organs, including appendicitis;
• acute inflammatory bowel diseases;
• severe abdominal pain associated with nausea and vomiting, which may be a sign of a more severe condition;
• severe dehydration;
• children under 10 years of age.

With caution:

Hepatic and / or renal insufficiency, advanced age.

Side effects

The most frequently reported adverse reactions during the use of the drug are spastic abdominal pain and diarrhea. From the gastrointestinal tract: spastic pain or discomfort in the abdomen, diarrhea (dehydration, muscle weakness, convulsions, low blood pressure), nausea, vomiting, a small amount of blood in the stool, discomfort in the rectum, colitis. The use of suppositories may be accompanied by pain and local irritation, especially with anal fissure or ulcerative proctitis. Nervous system disorders:  dizziness, fainting. These side effects that occur after the use of the drug are associated with a vasovagal reaction (i. e., due to intestinal spasm, tension during defecation). Immune system disorders: hypersensitivity, angioedema, anaphylactic reactions. Overdose: In case of overdose, possible:  diarrhea, dehydration, abdominal cramps, impaired water and electrolyte balance, hypokalemia. Like other laxatives, this drug can lead to chronic diarrhea, abdominal pain, hypokalemia, secondary hyperaldosteronism, and urolithiasis in a chronic overdose. Chronic laxative abuse can lead to renal tubule damage, metabolic alkalosis, and muscle weakness associated with hypokalemia. There is no treatment-specific antidote. Treatment should be symptomatic. Fluid replenishment and electrolyte balance adjustments may be required, which is especially important for elderly patients and children. You may also need to prescribe antispasmodics.

Interaction

Diuretics or glucocorticosteroids may increase the risk of electrolyte disturbances (hypokalemia) when taking excessively high doses of this drug.

A violation of the electrolysis balance (hypokalemia) can lead to hypersensitivity to the action of cardiac glycosides.

Special instructions

Patients should be advised that they may experience dizziness and/or fainting due to a vasovagal reaction (i. e., during a bowel spasm). Therefore, care should be taken when driving vehicles and engaging in other potentially dangerous activities that require increased concentration of attention and speed of psychomotor reactions.

Prolonged use can lead to fluid loss, electrolyte imbalance, hypokalemia, and dehydration, which can be accompanied by symptoms such as thirst and oliguria. Dehydration can be harmful to the body (for example, in renal failure, in elderly patients), so if the above symptoms occur, the drug should be discontinued and can only be resumed under the supervision of a doctor.

Patients may have traces of blood in the stool, but this phenomenon is usually mild and goes away on its own.

Patients taking this medication experienced dizziness and fainting. The analysis showed that these cases were associated with fainting during defecation (or fainting caused by straining during defecation), or with a vasovagal reaction to abdominal pain, which may be due to constipation, and not related to taking the drug.
