Composition

Active ingredient:  Loratadine -10 mgm Auxiliary substances: lactose, corn starch, gelatinized starch, magnesium stearate.

Pharmacological action

Lomilan – antihistamine, anti-allergic.

Pharmacodynamics

Loratadine belongs to antihistamines of systemic action, blockers of_H1-histamine receptors.

It has anti-allergic, antipruritic, and antiexudative effects. Reduces capillary permeability, prevents the development of tissue edema, and relieves smooth muscle spasms. The anti-allergic effect develops 30 minutes after taking the drug, reaches a maximum in 8-12 hours and lasts 24 hours.

It has no effect on the central nervous system and is not addictive.

Pharmacokinetics

Loratadine is rapidly and completely absorbed from the gastrointestinal tract. The presence of food slows down absorption. Cmax in serum is reached within 1 hour after use. The binding to plasma proteins is more than 95%. It is metabolized in the liver to form the active metabolite descarboethoxyloratadine.

It does not penetrate the BBB. T_1/2 is about 8 hours, increases in the elderly and in chronic alcoholism. It is excreted in the bile and kidneys.

In patients with chronic renal failure and hemodialysis, the pharmacokinetics practically do not change.

Indications

Prevention and treatment of the following diseases and conditions:

• seasonal and year-round allergic rhinitis;
• allergic conjunctivitis;
• allergic skin diseases (including chronic idiopathic urticaria);
• pseudoallergic reactions;
• allergic reactions to insect bites.

Contraindications

• hypersensitivity to any of the components of tablets or suspensions.

Side effects

Interaction

Concomitant use of inducers of microsomal oxidation (phenytoin, ethanol, barbiturates, rifampicin, tricyclic antidepressants) reduces the effectiveness of loratadine.

Possible interaction of the drug with inhibitors of cytochrome isoenzymes, such as CYP3A4 or CYP2D6 (ketoconazole, quinidine, itraconazole, erythromycin, fluoxetine), increases the concentration of Lomilan® in blood plasma, which may lead to increased side effects of the drug.

How to take, course of use and dosage

Inside, washed down with water or milk, it is possible to take it with food. If necessary, the tablet can be chewed.

Adults and children over 12 years of age — 2 dosage spoons (10 ml) 1 time a day; children from 2 to 12 years of age with a body weight of more than 30 kg-2 dosage spoons (10 ml) 1 time a day; with a body weight of less than 30 kg-1 dosage spoon (5 ml) 1 time a day.

Adults and children over 12 years of age — 10 mg (1 tablet) 1 time a day; children from 3 to 12 years of age with a body weight of more than 30 kg-10 mg (1 table) 1 time a day; with a body weight of less than 30 kg-5 mg (1/2 tablet) 1 time a day.

For patients with impaired liver function, it is necessary to prescribe a reduced daily dose of the drug for adults and children with a body weight of more than 30 kg, the dose is 10 mg (2 dosage spoons of suspension or 1 table). every other day.

Suspension is recommended for children under 12 years of age.

The duration of treatment depends on the duration of symptoms of the disease. If the patient’s condition does not improve within 3 days after starting treatment, then loratadine is ineffective.

No dose adjustment is required in elderly patients or patients with renal insufficiency.

Patients with severe renal insufficiency (Cl creatinine In adults and children from 6 years of age, the starting dose should be 10 mg (2 dosage spoons of suspension or 1 tablet) every other day; in children from 3 years of age — 5 mg (1 dosage spoon of suspension or 1/2 tablet) every other day.

Overdose

Symptoms:  headache, drowsiness, palpitations that may last for a long time.

Treatment:  when taking an excessive amount of Lomilan®, it is recommended to flush the stomach and prescribe an adsorbent (activated carbon). There is no specific antidote. Lomilan®is not eliminated by hemodialysis. To date, it is also unknown whether Lomilan® is eliminated by peritoneal dialysis. After performing emergency therapy, it is necessary to conduct medical supervision of the patient.
