Composition

1 ampoule (5 ml) contains: active ingredient magnesium sulfate — 1.25 g, auxiliary substance-water for injection.

Pharmacological action

Magnesium sulfate-sedative, antispasmodic.

Pharmacodynamics

When administered parenterally, it has a depressing effect on the central nervous system. Depending on the dose, there may be a sedative, hypnotic or anaesthetic effect.

Magnesium sulfate has a depressing effect on neuromuscular transmission. Reduces the excitability of the respiratory center, in large doses with parenteral use can cause respiratory paralysis. Lowers blood pressure, including due to the general calming effect (the effect is more pronounced in hypertension). Magnesium sulfate is excreted by the kidneys, and diuresis increases during excretion.

Antagonists of the magnesium ion are calcium ions. A decrease in the content of calcium in the blood is accompanied by an increase in the action of magnesium.

Indications

-correction and prevention of hypomagnesemia, including diabetes mellitus, complete parenteral nutrition, diuretics, muscle relaxants, chronic alcoholism, malabsorption;- arrhythmia (supraventricular tachycardia, ventricular tachycardia of the “pirouette” type);— arterial hypertension, including hypertensive crisis with signs of brain edema; – convulsive syndrome (leg cramps, muscle twitching, tremor and tetanic convulsions);- preeclampsia and eclampsia;- preterm birth, late pregnancy toxicosis;- poisoning with heavy metal salts: arsenic, tetraethyl lead, soluble barium salts.

Contraindications

-myasthenia gravis; – bradycardia; – severe arterial hypotension— – hypocalcemia;- prenatal period (2 hours before delivery)- severe renal impairment (creatinine clearance less than 20 ml / min);— hypersensitivity and hypermagnesemia.

How to take, course of use and dosage

Serum magnesium levels should be monitored before and during treatment. The dose of magnesium sulfate should be selected individually, depending on the patient’s needs and response to treatment. Excess of serum magnesium > 2.5 mmol / l should be avoided. Doses of magnesium sulfate are given in grams. They correspond to the following amount of solution: 1 g-4 ml (25%),2 g-8 ml (25%),3 g – 12 ml (25%),4 g – 16 ml (25%),5 g – 20 ml (25%),10 g – 40 ml (25%),15 g – 60 ml (25%),20 g – 80 ml (25%),30 g – 120 ml (25%),40 g – 160 ml (25%).1 ml of a 25% magnesium sulfate solution contains 1 mmol of magnesium.

Overdose

Symptoms:  possible respiratory depression, depression of the central nervous system, up to the development of anesthesia.

Treatment:  as an antidote for an overdose of magnesium sulfate, calcium preparations are used-calcium chloride or calcium gluconate.
