Composition

of 5 ml (1 tsp): – rimantadine hydrochloride 10 mg

Auxiliary substances:

sugar,

sodium alginate,

dye E 122,

purified water.

Pharmacological action

It has an antitoxic effect in acute respiratory viral infections and influenza caused by the type B virus.

The peculiarity of the drug is that rimantadine is fixed on the matrix of sodium alginate.

This allows Arvirem to provide prolonged circulation of the drug with a decrease in its toxicity, since in this dosage form rimantadine is not released immediately, which gives a gradual and prolonged entry into the blood.

This reduces the toxicity of rimantadine, improves the pharmacokinetics and mechanism of action of the drug.

Indications

Prevention and treatment of influenza and acute respiratory viral infections in children from 1 year to 7 years.

Use during pregnancy and lactation

The drug Orvirem is contraindicated for use during pregnancy and lactation.

Contraindications

-acute liver diseases— – acute and chronic kidney diseases;- thyrotoxicosis;— pregnancy; – lactation period; – children under 1 year of age;- hypersensitivity to the components of the drug Arvirem. With caution, the drug should be prescribed for epilepsy (including in the anamnesis).

Side effects

The drug Arvirem is usually well tolerated. Sometimes the following side effects are observed.

From the digestive system: nausea, vomiting, epigastric pain, flatulence, anorexia.

From the central nervous system: headache, dizziness, insomnia, neurological reactions, impaired concentration of attention.

Other: hyperbilirubinemia, allergic reactions (skin rash, pruritus, urticaria), asthenia.

Interaction

When used together, Orvirem ® reduces the effectiveness of antiepileptic drugs.

When taken simultaneously, adsorbents, astringents and enveloping agents reduce the absorption of rimantadine.

Drugs that acidify the urine (acetazolamide, sodium bicarbonate) increase the effectiveness of rimantadine due to a decrease in its excretion by the kidneys.

When combined with acetylsalicylic acid, paracetamol reduces the Cmax of rimantadine by 11%.

Cimetidine reduces the clearance of rimantadine by 18%.

How to take, course of use and dosage

During treatment, the drug Arvirem is taken orally (after meals), washed down with water, according to the following scheme:  

– children from 1 year to 3 years – on the 1st day,10 ml (2 teaspoons) of syrup (20 mg) 3 times a day. (daily dose-60 mg); on the 2nd and 3rd days-10 ml 2 times/day. (daily dose-40 mg), on the 4th day-10 ml 1 time/day. (daily dose-20 mg);

– children from 3 to 7 years – on the 1st day-15 ml (3 teaspoons) of syrup (30 mg) 3 times a day. (daily dose-90 mg); on the 2nd and 3rd days-3 teaspoons 2 times / day. (daily dose-60 mg), on the 4th day-3 teaspoons 1 time/day. (the daily dose is 30 mg).

For prevention, the drug Arvirem is prescribed:  

– children from 1 to 3 years – 10 ml (2 teaspoons) of syrup (20 mg) 1 time/day;

– children from 3 to 7 years – 15 ml (3 teaspoons) of syrup (30 mg) 1 time/day. within 10-15 days, depending on the source of infection.

The daily dose of rimantadine should not exceed 5 mg / kg of body weight.

Overdose

No cases of overdose have been identified so far.

Special instructions

Against the background of taking the drug Arvirem, an exacerbation of chronic concomitant diseases is possible.

With epilepsy, the risk of developing an epileptic seizure increases.

Orvirem syrup contains 60% sucrose, which should be taken into account when prescribing the drug to patients with diabetes mellitus.

Viruses that are resistant to the drug may appear.
