Treatment of postmenopausal osteoporosis to prevent spinal and hip fractures.
Osterepar is taken orally. The recommended dose is 70 mg once a week. To ensure optimal absorption, the drug should be taken in the morning on an empty stomach,2 hours (at least 30 minutes) before the first meal or liquid, washed down with plain drinking water. Other beverages, including mineral waters, food, and a number of medications may impair the absorption of alendronate.
In order to avoid local irritation of the oral mucosa and esophagus in the morning, immediately after getting out of bed, you should drink at least 200 ml of plain water, then take a tablet without chewing or letting it dissolve in your mouth, during the next 30 minutes you can not take a horizontal body position, after this period you should take breakfast. Do not take the pill in the morning, before getting out of bed, or in the evening, after going to bed. Â
Treatment with alendronate should be supplemented with calcium and vitamin D.
Elderly patients do not need to adjust the dose.
No dose adjustment is required for creatinine clearance > 35 ml/min. In case of renal failure with creatinine clearance
Hypersensitivity to any of the components of the drug;Â
anomalies of the esophagus and other factors that hinder the patency of the esophagus (achalasia, stricture, etc. );
the inability of the patient to remain upright, even sitting for 30 minutes; hypocalcemia, chronic renal failure (creatinine clearance < 35 ml/min),
pregnancy, lactation, infancy, vitamin D deficiency, severe disorders of mineral metabolism.
With caution: diseases of the digestive tract in the acute phase (dysphagia, esophageal diseases, gastritis, duodenitis, ulcers, serious diseases of the gastrointestinal tract in the previous 12 months, for example, peptic ulcer, gastrointestinal bleeding, surgery, except for operations on the spastic pylorus of the stomach).

Sodium alendronate is a bisphosphonate, a synthetic analog of pyrophosphate that binds bone hydroxyapatite. Being a non-hormonal specific inhibitor of osteoclast activity, it prevents bone resorption. It does not affect the processes of bone formation. Stimulates osteogenesis, restores a positive balance between bone resorption and recovery. Progressively increases bone mineral density (regulates phosphorus-calcium metabolism), promotes the formation of bone tissue of normal composition and structure.

The body as a whole:  hypersensitivity reactions, including urticaria and rare cases of angioedema; transient symptoms of myalgia, malaise, asthenia and, rarely, cases of fever, usually at the beginning of treatment; rarely — peripheral edema.
From the digestive tract: Â esophagitis, erosions and ulcers of the esophagus; rarely-esophageal stenosis or perforation, oropharyngeal ulcers, gastric or duodenal ulcers; rarely cases of localized osteonecrosis of the jaw, usually associated with tooth extraction and/or local infection, often with delayed healing, have been reported.
From the musculoskeletal system: Â bone, joint, and/or muscle pain, sometimes severe and rarely disabling.
Nervous system and sensory disorders: Â vertigo, vertigo; rarely-uveitis, scleritis or episcleritis.
From the side of the skin: Â rash (sometimes photosensitive), pruritus; rarely-severe skin reactions, including Stevens-Johnson syndrome and toxic epidermal necrolysis.
Calcium, antacids, certain oral medications, food, beverages, including mineral waters affect the absorption of alendronate. Medications can be taken orally no earlier than 1 hour after taking alendronate. Other types of interaction, with the exception of changes in absorption, are unlikely. Ranitidine increases the bioavailability of alendronate (clinical significance unknown). NSAIDs increase the adverse effects of alendronic acid. No specific drug interaction studies have been conducted, but studies with alendronate have included patients who were taking other oral medications at the same time. At the same time, they did not have any adverse reactions associated with the simultaneous use of other medications.

Symptoms: hypocalcemia, hypophosphatemia, side effects from the upper gastrointestinal tract (heartburn, esophagitis, gastritis, ulcers). Treatment: There is no specific treatment. It is recommended to take milk, antacids. To avoid irritation of the esophagus, do not induce vomiting, the patient should take an upright position (standing or sitting).
