Pharmacological properties

Pharmacodynamics

of Fluocortolone. Fluocortolone, when exposed to the skin, prevents the marginal accumulation of neutrophils, leads to a decrease in lymphokines and inhibits the migration of macrophages, helping to reduce the processes of infiltration, exudation and granulation.

Fluocortolone suppresses inflammatory and allergic skin reactions and relieves itching, burning and pain; reduces capillary dilation, interstitial edema and tissue infiltration.

Lidocaine. Lidocaine is a local anesthetic; analgesia is achieved by suppressing the formation and conduction of nerve impulses along afferent nerve fibers by depolarizing sodium channels.

Pharmacokinetics: the therapeutic effect is achieved with a low level of active components in the blood plasma.

Fluocortolone pivalate. After a single rectal application of 1 g of cream or use of one suppository, the absorption of glucocorticosteroid was maximum 5% of the applied or administered amount of the drug.

During the period of daily use of 2 suppositories 3 times a day for 4 weeks, the content of fluocortolone in blood plasma did not reach the level that has a systemic effect.

Fluocortolone pivalate is hydrolyzed by esterases, enzymes of the inflammatory focus, to form fluocortolone,11-ketofluocortolone, and trimethylacetic acid.

The plasma half-lives of fluocortolone and its metabolites after rectal use were approximately 1.3 and 4 hours, respectively.

Fluocortolone is excreted from the body in the form of metabolites mainly together with the urine. Lidocaine. The absorption and bioavailability of lidocaine after rectal use of the cream and suppository is about 30% and 24%, respectively.

The plasma half-life of lidocaine is 1-2 hours. In humans, lidocaine is metabolized by oxidative N-dealkylation, hydrolysis of the amide bond, and hydroxylation of the aromatic ring to form 4-hydroxy-2,6-xylidine, which is the main metabolite and 70% of the drug is excreted in the urine as this metabolite.

Indications

Hemorrhoids, Proctitis.

Contraindications

Tuberculosis and syphilitic processes in the area of drug application.

Viral diseases (for example, chickenpox, vaccination reaction, shingles) in the area of application of the drug.

Treatment of children and adolescents is not recommended, due to the lack of data on clinical trials.

Hypersensitivity to the components of the drug

Side effects

With prolonged treatment with both Relief Pro cream and suppositories (more than four weeks), there is a risk of developing local skin changes, such as atrophy, striae or telangiectasia. Burning sensation: 1-10%, rarely-irritation and allergic reactions-0.1-1%.

Interaction

Patients receiving antiarrhythmic medications should use lidocaine with caution. When lidocaine is co-administered with antiarrhythmic drugs, the QT interval may be prolonged and, in very rare cases, AV block or ventricular fibrillation may develop.

How to take, course of use and dosage

Relief Pro is recommended for use after defecation. Before using the drug, you should perform hygiene of the anus (anus).

One suppository is injected deep into the anus twice a day, in the morning and in the evening, but in severe cases of the disease, one suppository is administered three times a day for the first three days. With stable improvement, in many cases it is enough to enter one suppository per day or every other day.

However, if the hemorrhoids are very inflamed and therefore painful, it is recommended to start treatment with a cream. Protruding hemorrhoids should be liberally lubricated with cream, gently setting them back with your finger.

Overdose

If the drug is accidentally taken orally (for example, if you swallow several grams of cream or more than one suppository), the most severe symptoms may occur from the cardiovascular system (cardiac depression, cardiac arrest) and the central nervous system (convulsions, respiratory depression, respiratory arrest), depending on the dose.

Special instructions

In the presence of fungal infections, appropriate antifungal therapy is required in addition to Relief Pro. It is necessary to avoid getting Relief Pro inside or in contact with eyes. After applying the cream, it is recommended to wash your hands thoroughly.
