Pharmacodynamics
Antimicrobial bacteriostatic agent. The mechanism of action is due to antagonism with paraaminobenzoic acid (PABA) and competitive inhibition of dihydropteroate synthetase, which leads to a violation of the synthesis of tetrahydrofolic acid, necessary for the synthesis of purines and pyrimidines.
Inhibits the growth of Escherichia coli and reduces the synthesis of thiamine, riboflavin, nicotinic acid in the intestines. SULGIN has a bacteriostatic effect on Streptococcus spp., Staphylococcus spp., Escherichia coli, Shigella dysenteriae, Proteus vulgaris.
Pharmacokinetics
After oral use, the drug is slightly absorbed from the digestive tract, resulting in a high bacteriostatic concentration in the intestine.
From the digestive system: nausea, vomiting.
From the urinary system: crystalluria.
Allergic reactions: urticaria, exfoliative dermatitis.
Other services: hypovitaminosis B, leukopenia, neuritis.
Reduces the reliability of oral contraception.
Benzocaine and procaine reduce antimicrobial activity (PABA is released during hydrolysis).
Nonsteroidal anti-inflammatory drugs, hypoglycemic drugs (sulfonylurea derivatives), phenytoin and indirect anticoagulants increase the severity of side effects of the drug.
Ascorbic acid, hexamethylenetetramine increase the risk of crystalluria.
Myelotoxic drugs increase the manifestations of hematotoxicity of the drug.
In acute dysentery,2 courses of treatment are carried out.
The first course: on day 1 and 2 – 1 g every 4 hours (total 6 g / day), day 3 and 4 – 1 g every 6 hours (4 g/day), day 5 and 6 – 1 g every 8 hours (3 g/day).
If necessary, after 5-6 days, the second course is carried out: 1 and 2 days – 1 g every 4 hours, at night – after 8 hours (only 5 g/day),3 and 4 days – 1 g after 4 hours (4 g/day), at night they do not give,5 days – 3 g/day. Children older than 3 years – 0.5 g 4 times a day for 7 days.
In the treatment of other infections: adults-inside,1-2 g every 4-6 hours; in the next 2-3 days – half doses. Children over 3 years of age – on the first day,0.1 g / kg / day in equal doses every 4 hours, on the following days-0.25-0.5 g every 6-8 hours.
For the prevention of postoperative intestinal complications – 0.05 g / kg every 8 hours, for 5 days before and 7 days after surgery.
The highest doses for adults: single – 2 g, daily – 7 g.
In order to prevent crystalluria during treatment, patients should receive a plentiful drink (2-3 liters/day). Early discontinuation of treatment or insufficient doses can lead to the emergence of resistant strains of pathogens. During the treatment period, it is recommended to take B vitamins.
In a dry place at a temperature not exceeding 30 °C. Keep out of reach of children.
