Composition

Active ingredients:

mupirocin 20 mg,

betamethasone (in the form of dipropionate) 500 mcg.

Pharmacological action

Combined preparation for external use, the action of which is due to the components included in its composition.

Mupirocin is an antibiotic produced by the microorganism Pseudomonas fluorescens. Mupirocin suppresses bacterial cell protein synthesis by reversibly binding to isoleucyl transport RNA synthetase, which prevents isoleucine from being incorporated into the protein chains under construction.

When used in minimal inhibitory concentrations, mupirocin has a bacteriostatic, and in higher concentrations – a bactericidal effect. Due to its specific mechanism of action and unique chemical structure, there is no cross-resistance with other antibiotics. With proper use of mupirocin, the risk of developing resistant strains of microorganisms is small.

Betamethasone is a glucocorticosteroid for external use. It has local anti-inflammatory, anti-allergic, anti-edematous and antiproliferative effects. It has a stronger vasoconstrictor effect than other fluoro derivatives of glucocorticosteroids; systemic effects are insignificant due to limited absorption through the skin.

When exposed to the skin, the marginal accumulation of neutrophils is prevented, which leads to a decrease in exudation, cytokine production, and inhibition of macrophage migration, ultimately leading to a decrease in infiltration and granulation processes.

Indications

Treatment of atopic dermatitis, diffuse neurodermatitis, contact dermatitis (simple and allergic), exfoliative dermatitis, urticaria, herpetiform dermatitis, seborrheic dermatitis, eczema, psoriasis complicated by secondary bacterial infections.

Use during pregnancy and lactation

The drug is contraindicated for use during pregnancy and lactation.

Contraindications

• cutaneous manifestations of syphilis;
• tuberculosis of the skin;
• bacterial, viral, fungal skin diseases;
• venous leg ulcers associated with varicose veins;
• skin cancer, nevus, sebaceous cyst, melanoma, hemangioma, a xanthoma, sarcoma;
• rosacea;
• acne vulgaris;
• post-vaccination skin reactions;
• pregnancy and breastfeeding;
• children up to age 12 years;
• hypersensitivity.

With caution: Macrogol (polyethylene glycol) can be absorbed through the wound surface or through other skin lesions and excreted by the kidneys. Macrogol-based ointments should not be used on large surfaces in conditions accompanied by increased absorption of polyethylene glycol, and especially in moderate to severe renal failure.

For patients with diabetes mellitus, glaucoma, tuberculosis, cataracts, long-term use of ointment on large areas of the skin is not recommended. It should be used with caution in patients with atrophy of subcutaneous tissue, especially in the elderly.

How to take, course of use and dosage

Recommended dosage: apply a thin layer of ointment to the affected area 2-3 times a day for 5-14 days.

Use only for the treatment of affected areas with a length of no more than 10 cm or an area of no more than 100 cm2.

If patients do not show a clinical response within 3-5 days, they should be re-examined.

Overdose

Symptoms:  reversible suppression of the hypothalamic-pituitary-adrenal system, manifestations of Cushing’s syndrome, hyperglycemia, glucosuria.

Treatment:  symptomatic.

Special instructions

Avoid contact with the ointment in the eyes, on the mucous membranes, as well as on open wounds and burns.

As with other antibacterial agents, prolonged use of the ointment can lead to excessive growth of organisms that are immune to its action, including fungi.

Given the presence of a strong glucocorticosteroid in the drug, the drug should be used in the acute period of the disease, for a short time, on a small surface of the skin.

Do not use Supirocin B on the skin of the face due to the possibility of telangiectasia, perioral dermatitis.

On the skin in the armpit and groin area, the drug should be used only in cases of extreme necessity.

Do not use the drug under an occlusal dressing, because this can lead to atrophy of the epidermis, the appearance of striae, the development of superinfection.

Macrogol-based ointments should not be used on large surfaces in conditions accompanied by increased absorption of polyethylene glycol, and especially in moderate to severe renal failure.
