Teturam inhibits acetaldehyde dehydrogenase, which is involved in the metabolism of ethanol; increases the concentration of acetaldehyde, which is a metabolite of ethanol and causes a number of unpleasant sensations after its use (“hot flashes” of blood to the skin of the face, nausea, vomiting, feeling unwell, tachycardia, lowering blood pressure).
As a result, a conditioned reflex reaction of disgust to the taste and smell of ethanol is developed.
The duration of action of oral tablets is 48 hours. Application in the form of implants creates a constant concentration of the drug in the body. Disulfiram-ethanol reactions may occur within 5-9 months after implantation.
In the case of ethanol intake, lipids are mobilized, blood vessels dilate and blood flow at the implantation site increases, as a result of which the supply of disulfiram to the blood increases.

During treatment with Teturam, a bitter taste in the oral cavity, headache, dizziness, memory disorder, insomnia, and anxiety may appear.
The most common side effects of Teturam are allergic reactions – urticaria, skin rash, angioedema, bronchospasm, dermatitis.
When Teturam tablets are implanted, rejection may occur, which manifests itself in the form of collapse, arrhythmia, brain edema, suppuration, and heart attack.
Contraindicated combinations
Alcohol: an intolerance reaction (hot flashes, erythema, vomiting, tachycardia). Avoid taking alcoholic beverages and medicines containing alcohol.
Unwanted combinations
Isoniazid: behavioral and coordination disorders.
Nitro-5-imidazoles (metronidazole, ordinazole, secnidazole, tinidazole): delirious disorders, confusion.
Phenytoin. : a significant and rapid increase in the level of phenytoin in plasma with toxic symptoms (suppression of its metabolism).
If the combination cannot be avoided, clinical monitoring and monitoring of the drug’s plasma concentrations should be performed before and after treatment with teturam.
Combinations that require caution
Warfarin (and other oral anticoagulants): increased effect of oral anticoagulants and risk of bleeding (reduced breakdown of warfarin in the liver). More frequent monitoring of warfarin concentrations and dose adjustment of anticoagulants are recommended within 8 days after teturam withdrawal.
Theophylline: Teturam inhibits theophylline metabolism. As a result, the dose of theophylline should be adjusted (reduced dosage), depending on the clinical symptoms and the concentration of the drug in plasma.
Benzodiazepines: Teturam may potentiate the sedative effect of benzodiazepines by inhibiting their oxidative metabolism (especially chlordiazepoxide and diazepam). The dosage of benzodiazepine should be adjusted according to the clinical manifestations.
Tricyclic antidepressants: it is possible to increase the reaction of alcohol intolerance (especially if patients take alcoholic beverages while taking teturam).
Treatment is prescribed after a thorough examination of the patient and warnings about the consequences and complications. The drug is taken orally no more than 500 mg 1 time a day according to an individual scheme.
After 7-10 days, a teturamalcohol test is performed (20-30 ml of 40% vodka after taking 500 mg of the drug), with a weak reaction, the alcohol dose is increased by 10-20 ml (the maximum dose of vodka is 100-120 ml).
The test is repeated after 1-2 days in the hospital and after 3-5 days on an outpatient basis, with correction of alcohol and/or drug doses as necessary. In the future, you can use a maintenance dose of 150-200 mg per day for 1-3 years.
Symptoms: the combination of teturam-ethanol can cause depression of consciousness up to coma, cardiovascular collapse, and neurological complications.
Treatment: symptomatic.
Patients should be warned about the danger of an alcohol intolerance reaction.
Teturam should be taken with caution in patients with renal insufficiency or hypothyroidism, especially at the risk of possible combination with alcohol.
