Pharmacological action
Sodium and chlorine ions are the most important inorganic components of extracellular fluid.
They maintain the appropriate osmotic pressure of blood plasma and extracellular fluid.
During dehydration, an isotonic solution makes up for the lack of fluid in the body.
Hypertonic sodium chloride solution when used intravenously provides correction of osmotic pressure of extracellular fluid and blood plasma.
Also, sodium chloride is used in ophthalmology – when applied topically, it has an anti-edematous effect.
Indications
Isotonic solution: dehydration of various origins. To maintain blood plasma volume during and after surgery. As a solvent for various preparations.
Hypertonic solution: disorders of water and electrolyte metabolism: deficiency of sodium and chlorine ions; hypoosmolar dehydration of various origins (due to prolonged vomiting, diarrhea, burns; with gastric fistula, pulmonary bleeding, intestinal bleeding).
Eye drops and ointment: corneal irritation in inflammatory and allergic diseases (as part of combination therapy).
Use during pregnancy and lactation
The solution can be administered during pregnancy and lactation.
Contraindications
- Hypernatremia.
- Acidosis.
- Hyperchloremia.
- Hypokalemia.
- Extracellular hyperhydration.
- Circulatory disorders that threaten edema of the brain and lungs.
- Brain edema.
- Pulmonary edema.
- Acute left ventricular failure.
- Concomitant use of high-dose glucocorticosteroids.
With caution:
- Decompensated chronic heart failure.
- Arterial hypertension.
- Peripheral edema.
- Preeclampsia.
- Chronic renal failure:
- Oliguria.
- Anuria.
Aldosteronism and other conditions associated with sodium retention in the body.
Use in patients with impaired renal function
Caution should be exercised when using large amounts of sodium chloride in patients with impaired renal excretory function.
Side effects
Possible: Â nausea, vomiting, diarrhea, stomach cramps, thirst, lacrimation, sweating, fever, tachycardia, hypertension, renal dysfunction, edema, shortness of breath, headache, dizziness, restlessness, weakness, twitching and muscle hypertonicity.
With external and topical application, side effects have not yet been established.
Interaction
Compatible with colloidal hemodynamic blood substitutes (mutual effect enhancement).
When mixing with other medicinal products, it is necessary to visually monitor compatibility (however, invisible and therapeutic incompatibilities are possible).
How to take, course of use and dosage
Isotonic sodium chloride solution is administered intravenously, subcutaneously and in enemas, and is also used to wash wounds, eyes, and nasal mucosa. More often administered intravenously, depending on the clinical situation-up to 3 l / day.
Hypertonic sodium chloride solution is administered intravenously. A single dose for intravenous jet use can be 10-30 ml. In conditions requiring immediate replenishment of sodium and chlorine ions, the drug is administered intravenously at a dose of 100 ml.
Topically and topically applied depending on the dosage form used and the treatment regimen.
Special instructions
Use with caution large amounts of sodium chloride in patients with impaired excretory function of the kidneys, with hypokalemia. The introduction of large amounts of the solution can lead to chloride acidosis, hyperhydration, and increased excretion of potassium from the body.
Hypertonic solution is not used subcutaneously or intravenously.
With prolonged use, it is necessary to monitor the concentration of electrolytes in plasma and daily diuresis.
The temperature of the infusion solution should be 38°C.
