Composition

Pharmacological action

Spazmonet is an antispasmodic drug.

Pharmacodynamics

Drotaverin is a myotropic antispasmodic, similar in chemical structure and pharmacological properties to papaverine, but has a stronger and longer-lasting effect.

Reduces the influx of calcium ions into smooth muscle cells.

Reduces the tone of smooth muscles of internal organs and intestinal motility, dilates blood vessels. It does not affect the autonomic nervous system, does not penetrate the central nervous system.

Its direct effect on smooth muscles allows it to be used as an antispasmodic when medications from the group of M-holinoblockers (angle-closure glaucoma, prostatic hypertrophy) are contraindicated.

Pharmacokinetics

When taken orally, absorption is high. Bioavailability is 100%.

Evenly distributed throughout the tissues, penetrates into smooth muscle cells.

The maximum concentration in the blood reaches in 2 hours. The relationship with plasma proteins is 80-90%.

The elimination half-life is 7-12 hours. Drotaverine is actively metabolized in the liver, mainly excreted by the kidneys, to a lesser extent with bile. It does not cross the blood-brain barrier.

Indications

• renal colic
• painful menstruation
• biliary dyskinesia,
• biliary colic,
• cholecystitis.
• pyelitis;
• spastic constipation, spastic colitis, proctitis, tanismak;
• the pilorospazme, gastroduodenitis, gastric ulcer and 12 duodenal ulcer (in the acute phase, in the complex therapy for relief of pain syndrome spastic nature);
• in disease,
• spasm of the peripheral, coronary arteries, and spasm of cerebral arteries, accompanied by headaches;
• the algomenorrhea;
• to reduce the excitability of the uterus during pregnancy (threatened abortion, threatened preterm delivery); spasm of the cervix during childbirth, prolonged opening of the throat, postpartum contractions.

To prevent the occurrence of spasm: during some instrumental studies, cholecystography.

Use during pregnancy and lactation

The use of the drug during pregnancy and lactation is not contraindicated.

Use with caution during pregnancy (I trimester) and children under 6 years of age.

Contraindications

• hypersensitivity,
• severe renal and hepatic insufficiency,
• chronic heart failure,
• atrioventricular block II and III degrees,
• cardiogenic shock,
• arterial hypotension.

Spazmonet tablets contain lactose, so patients with congenital galactose intolerance, lactose deficiency or glucose-galactose malabsorption should not take the drug.

With caution:  severe coronary artery atherosclerosis, angle-closure glaucoma, and prostatic hyperplasia.

Side effects

• palpitations
• abnormal heart rhythm
• allergic reactions.

Interaction

When used concomitantly, drotaverine may weaken the antiparkinsonian effect of levodopa.

Increases the effect of papaverine, bendazole and other antispasmodics (including m-holinoblokatorov), reduces blood pressure caused by tricyclic antidepressants, quinidine and procainamide. Phenobarbital increases the severity of the antispasmodic effect of drotaverine.

Drotaverine reduces the spasmodic activity of morphine.

How to take, course of use and dosage

Inside, with a small amount of liquid.

Children under 6 years of age: in a single dose of 10-20 mg, the frequency of use 1-2 times a day, the maximum daily dose is 120 mg.

Children aged 6-12 years – 1/2 tablet 2 times a day (20-40 mg drotaverine); the maximum daily dose is 200 mg.

Adults and children over 12 years of age-1-2 tablets 3 times a day (120-240 mg of drotaverine).

Overdose

In large doses, it disrupts atrioventricular conduction, reduces the excitability of the heart muscle, and can cause cardiac arrest and paralysis of the respiratory center.

Special instructions

Use Spazmonet with caution in patients with severe atherosclerosis of the coronary vessels.

Influence on the ability to drive a car and work with mechanisms.

During the period of use of the drug, it is necessary to refrain from driving vehicles and engaging in potentially dangerous activities that require concentration of attention and speed of psychomotor reactions.
