Pharmacological action
Bacteriostatic broad-spectrum antibiotic, left-and right-rotating isomer of chloramphenicol. Disrupts the process of protein synthesis in a microbial cell (having high lipophilicity, it penetrates the bacterial cell membrane and reversibly binds to the 50S subunit of bacterial ribosomes, in which the movement of amino acids to growing peptide chains is delayed, which leads to a violation of protein synthesis). Active against most strains of gram-positive and gram-negative microorganisms resistant to penicillin, tetracyclines, and sulfonamides. Promotes cleansing and healing of burn wounds and trophic ulcers, accelerates epithelialization.
Pharmacokinetics
The degree of systemic action after application to the skin is unknown.
Indications
Infected wounds in the second phase of the wound process (absence of pus, necrotic tissues) caused by sensitive microorganisms: burns of 2-3 degrees, long-term non-healing trophic ulcers.
Use during pregnancy and lactation

Contraindications
Hypersensitivity to chloramphenicol and other excipients, neonatal period (up to 4 weeks), inhibition of bone marrow hematopoiesis, acute intermittent porphyria, glucose-6-phosphate dehydrogenase deficiency, hepatic and / or renal failure, skin diseases (fungal lesions, psoriasis, eczema), phase I of the wound process (due to lack of osmotic activity).
Side effects
Allergic reactions – skin rash, angioedema.
Hematopoietic disorders: reticulocytopenia, leukopenia, granulocytopenia, thrombocytopenia, erythropenia, aplastic anemia, agranulocytosis.

Interaction
When used concomitantly with erythromycin, clindamycin, or lincomycin, a mutual weakening of the action is possible due to the fact that chloramphenicol [D, L] can displace these drugs from the bound state or prevent their binding to the 50S subunit of bacterial ribosomes. Reduces the antibacterial effect of penicillins and cephalosporins.
How to take, course of use and dosage
Externally. After surgical treatment of wounds and burns, liniment is applied directly to the wound surface, after which a sterile gauze dressing is applied or liniment is applied to the dressing material, and then to the wound. Swabs with liniment loosely fill the cavities of purulent wounds after their surgical treatment, and gauze turunds with the drug are inserted into the fistulous passages.
Multiplicity of application – 1 time in 1-3 days, depending on the dynamics of healing.

Special instructions
In the course of treatment, systematic monitoring of the peripheral blood picture is necessary. When applied to large surfaces with simultaneous intake of ethanol, disulfiram-like reactions may develop (skin hyperemia, tachycardia, nausea, vomiting, reflex cough, convulsions).
