Active ingredient: Chloramphenicol [D, L] (Synthomycin) – 100 mg; Excipients: Castor oil-200.0 mg, Emulsifier no. 1 — 30,0-50,0 alcohol content of 100 mg, Sorbic acid — 2.0 mg, Ethanol — 8.1 mg in terms of 96% alcohol, Carmellose sodium-18.4 mg in terms of 450-500 degree of polymerization, water-up to 1 g
Pharmacological action
The Active ingredient of the drug is chloramphenicol — a broad-spectrum antibiotic with high antibacterial activity against the causative agent of wound infection and various forms of purulent-inflammatory processes. Chloramphenicol is a bacteriostatic antibiotic that disrupts the process of protein synthesis in a microbial cell (having high lipophilicity, it penetrates the bacterial cell membrane and reversibly binds to the 50S subunit of bacterial ribosomes, as a result of which the movement of amino acids to growing peptide chains is delayed, which leads to disruption of the formation of peptide bonds and protein synthesis). Active against most strains of gram-positive and gram-negative microorganisms resistant to penicillin, tetracyclines, and sulfonamides. Promotes cleansing and healing of burn wounds and trophic ulcers, accelerates epithelialization. Pharmacokinetics: the degree of absorption into the systemic circulation after application of the drug to the skin, wounds or mucous membranes is unknown.
Indications
- bacterial skin infections caused by sensitive microorganisms, including infected burns (superficial and deep-bordered)
- bedsores
- trophic ulcers
- of wounds (phase II of the wound process: absence of pus, necrotic tissues – for liniment devoid of osmotic activity)
- boils
- cracked nipples in nursing women.
Contraindications
Individual intolerance, inhibition of bone marrow hematopoiesis, acute intermittent porphyria, glucose-6-phosphate dehydrogenase deficiency, hepatic and / or renal insufficiency, skin diseases (fungal lesions, psoriasis, eczema), pregnancy, lactation, neonatal period (up to 4 weeks). Due to the lack of osmotic activity (base: oil, emulsifier), it is not recommended to use in the first phase of the wound process (profuse suppuration, severe tissue edema, pain and the presence of necrotic tissues). Use caution in early childhood, in patients who have previously received treatment with cytostatic drugs or radiation therapy.
Side effects
Possible allergic reactions: skin rash, angioedema. Hematopoietic disorders: rare-reticulocytopenia, leukopenia, granulocytopenia, thrombocytopenia, erythropenia, aplastic anemia, agranulocytosis.
Interaction
Barbiturates accelerate biotransformation, reduce the concentration and duration of the effect.
Reduces the antibacterial effect of penicillins and cephalosporins.
When used concomitantly with erythromycin, clindamycin, lincomycin, there is a mutual weakening of the action.
When drinking alcohol at the same time, disulfiram-like reactions are possible.
How to take, course of use and dosage
Liniment is applied externally.
After surgical treatment of wounds and burns, Synthomycin liniment is applied directly to the wound surface, after which a sterile gauze dressing is applied, or Synthomycin liniment is applied to the dressing material, and then to the wound.
Swabs with liniment loosely fill the cavities of purulent wounds after their surgical treatment, and gauze turunds with liniment of Synthomycin are inserted into the fistulous passages.
In the treatment of wounds and burns in the 2nd phase-1 time in 1-3 days, depending on the dynamics of wound healing.
Special instructions
In the course of treatment, systematic monitoring of the peripheral blood picture is necessary.
There were no adverse effects on the fetus during pregnancy and breastfeeding.
When applied to large surfaces with simultaneous intake of ethanol, disulfiram-like reactions may develop (skin hyperemia, tachycardia, nausea, vomiting, reflex cough, convulsions).
