Composition

1 ml of eye drops contains:
active ingredients:
timolol maleate (in terms of timolol) – 5.0 mg
. excipients:
disodium phosphate dodecahydrate,
sodium dihydrogen phosphate monohydrate,
disodium edetate dihydrate,
sodium chloride,
benzalkonium chloride,
sodium hydroxide,
purified water.
Pharmacological action
Non-selective beta-blocker. It has antianginal, antihypertensive and antiarrhythmic effects. Reduces the automatism of the sinus node, reduces heart rate, slows down AV conduction, reduces contractility and the need for oxygen in the myocardium.
When applied topically in ophthalmology, it reduces both normal and elevated intraocular pressure by reducing the formation of intraocular fluid and improving its outflow, and does not affect accommodation and pupil size.
Indications
For use in ophthalmology: increased intraocular pressure, chronic open-angle glaucoma, angle-closure glaucoma (as an additional agent in combination with myotics), secondary glaucoma (including aphakic), congenital glaucoma (if other therapeutic measures are ineffective).
Contraindications
AV block II and III degrees, sinoatrial block, bradycardia, SSR, hypotension, chronic heart failure IIB-III stages, acute heart failure, vasomotor rhinitis, Raynaud’s disease and other obliterating vascular diseases, metabolic acidosis, lactation.
Side effects
From the side of the visual organ: topical application may cause irritation and hyperemia of the conjunctiva, eyelid skin, burning and itching in the eyes, lacrimation, photophobia, edema of the corneal epithelium, spot surface keratopathy, corneal hypesthesia, diplopia, ptosis, dry eyes. When performing fistulizing anti-glaucoma operations, retinal detachment may develop in the postoperative period.
With systemic use, heart failure, bradycardia, AV block, hypotension; headache, sleep disorders, nightmares, asthenia, agitation, depression, paresthesia and cold extremities; nausea, vomiting, diarrhea; shortness of breath, bronchospasm; muscle weakness; allergic skin reactions, exacerbation of psoriasis, dry conjunctiva are possible.
Interaction
When used concomitantly with Timolol, epinephrine, pilocarpine, and systemic beta-blockers enhance its effect.
With the simultaneous use of Timolol with reserpine, severe bradycardia or arterial hypotension may develop (the combination requires careful medical supervision).
Concomitant use of Timolol with calcium channel blockers or cardiac glycosides may result in impaired AV conduction, acute left ventricular failure, or hypotension.
Concomitant use of Timolol increases the effect of muscle relaxants (the drug should be discontinued 48 hours before the expected general anesthesia using peripheral muscle relaxants).
When using local anesthetics, you should consider the possibility of increasing the risk of their undesirable effects on the cardiovascular system.
During treatment with Timolol, it is not recommended to take ethanol in order to avoid a sharp decrease in blood pressure.
How to take, course of use and dosage
Adults and children over the age of 10 years are instilled into the conjunctival sac 1 drop of 0.25% solution 2 times a day. In case of insufficient effectiveness – 1 drop of 0.5% solution 2 times / day. With normalization of intraocular pressure, the maintenance dose of the drug is 1 drop of 0.25% solution 1 time/day.
Overdose
When applied topically at the recommended dose, no symptoms of overdose were observed.
If accidentally ingested, the following symptoms may develop: nausea, vomiting, dizziness, headache, decreased blood pressure, bronchospasm, bradycardia.
Treatment: conduct symptomatic therapy. To eliminate severe bradycardia or bronchospasm – IV isoprenaline, for the treatment of arterial hypotension – dobutamine.
Special instructions
It is used with caution in patients with impaired liver, kidney function, diabetes mellitus (especially labile course). With prolonged use, it increases the level of triglycerides in the blood plasma.
The safety and efficacy of its use in children has not been studied.
In ophthalmology, it is used for a long time, so during the treatment period, the cornea should be examined at least once every 6 months, tear production function and the state of visual fields should be monitored.
Influence on the ability to drive motor vehicles and manage mechanisms
During the treatment period, you should refrain from engaging in potentially dangerous activities that require increased attention and rapid psychomotor reactions.
When used in ophthalmology immediately after instillation, visual acuity may decrease and psychomotor reactions may slow down, so you should refrain from engaging in potentially dangerous activities that require increased attention for 30 minutes.
