Pharmacological action
Pharmacodynamics : Xylometazoline (an imidazole derivative) is an alpha-adrenostimulator that narrows the blood vessels of the nasal mucosa, eliminating edema and hyperemia of the mucous membrane. Facilitates nasal breathing in rhinitis. Hyaluronic acid, which is part of the preparation, has a moisturizing effect when used in nasal medicines due to its ability to maintain the mucous membranes in a moistened state and creates optimal conditions for recovery processes. The action usually starts within 5-10 minutes. Pharmacokinetics: When applied topically, it is practically not absorbed, and plasma concentrations are so low that they cannot be determined by modern analytical methods. No pharmacokinetic studies have been performed in humans.
Indications
* Reduction of swelling of the nasal mucosa in rhinitis, including allergic rhinitis, acute respiratory infections with rhinitis, pollinosis. * Relief of nasal discharge in paranasal sinusitis and middle ear inflammation caused by the common cold. * Preparation of patients for diagnostic manipulations in the nasal passages. “Tizin® Expert”, nasal spray dosed 0.05% is intended for children aged 2 to 6 years. “Tizin® Expert”, nasal spray dosed 0.1% is intended for adults and children over 6 years of age.
Use during pregnancy and lactation
Xylometazoline should not be used during pregnancy, as the effect of this drug on the fetus has not been studied in clinical studies. The drug should not be used during breast-feeding, as it is not known whether the Active ingredient is excreted in breast milk.
Contraindications
* hypersensitivity to any of the components of the drug; • atrophic rhinitis, thyrotoxicosis; • newborns and children under 2 years of age (for nasal spray dosed 0.05%); children under 6 years of age (for nasal spray dosed 0.1%);• patients with severe cardiovascular diseases (for example, coronary heart disease, angina pectoris, tachycardia, arterial hypertension, severe atherosclerosis);• patients with a history of surgical interventions on the meninges• patients receiving monoamine oxidase inhibitors (MAOIs) or other drugs that may cause an increase in blood pressure; * patients with high intraocular pressure, especially with angle-closure glaucoma;• condition after transsphenoidal pituitary surgery.
Side effects
Respiratory tract “Tizin® Expert” may cause temporary mild irritation of the nose (burning sensation) in susceptible people, sneezing. Often (>1–> Prolonged or too frequent use of xylometazoline, or its use in high doses, can lead to a burning sensation in the nose or dryness of the mucous membrane, as well as reactive nasal congestion, with the development of drug-induced rhinitis, paresthesia, sneezing, hypersecretion. This effect can be observed even 5-7 days after the end of treatment, and with prolonged use it can cause chronic inflammation of the mucous membrane with the formation of crusts (dry rhinitis). Interaction
Concomitant use of MAO inhibitors such as tranylcypromine or tricyclic antidepressants may lead to increased blood pressure due to the cardiovascular effects of these substances.
How to take, course of use and dosage
Adults and children over 6 years of age – 0.1% * Up to 3 times a day, one dose of the drug is administered in the form of a 0.1% dosed spray in each nostril. The dose depends on the individual sensitivity of the patient and the clinical effect. Children aged 2-6 years – 0.05%• Unless otherwise prescribed, use one dose of the drug in the form of a 0.05% dosed spray in each nostril 1-2 times a day. Xylometazoline in the form of a dosed spray should not be used for more than 5-7 days, unless the doctor recommended a different duration of treatment. After the end of therapy, the drug can be prescribed again only after a few days. Always consult your doctor about the duration of use in children. In the case of chronic rhinitis, the drug ” Tizin® Expert ” 0.05% and 0.1% can be used only under the supervision of a doctor, taking into account the risk of atrophy of the nasal mucosa. Method of applicatio Nmake off the protective cap. Before the first application, press the spray nozzle several times until a uniform cloud of “fog” appears. The bottle is ready for further use. When applying, tap once. The drug should be inhaled through the nose. If possible, hold the spray bottle vertically. Do not spray horizontally or downwards. After use, close the bottle with a cap.
Overdose
Symptoms: overdose or accidental ingestion of the drug can lead to the following symptoms: dilated pupils, nausea, vomiting, cyanosis, fever, convulsions, tachycardia, arrhythmia, collapse, cardiac arrest, hypertension, pulmonary edema, respiratory disorders, mental disorders. In addition, the following symptoms may occur: suppression of central nervous system function, accompanied by drowsiness, decreased body temperature, bradycardia, hypotension, apnea, and coma. Treatment: the use of activated charcoal, gastric lavage, artificial respiration with the introduction of oxygen. To reduce blood pressure, phentolamine 5 mg in saline solution is used intravenously slowly or 100 mg orally. Vasopressors are contraindicated. If necessary, apply antipyretics and anticonvulsants.

Prolonged use (more than 7 days) and an overdose of sympathomimetics that have a decongestive effect can lead to reactive hyperemia of the nasal mucosa. As a result, there is difficulty in nasal breathing, which leads to the fact that the patient begins to use the drug repeatedly or even constantly. This can lead to chronic swelling (drug-induced rhinitis), and eventually even to atrophy of the nasal mucosa (ozen).
In milder cases, to improve the patient’s condition, you can first stop injecting sympathomimetic into one nostril, and after reducing complaints, continue to inject it into the other nostril to at least partially provide nasal breathing.

It is not recommended to use the drug continuously for more than 7 days.
