Pharmacological action
Vinpocetine is a drug that improves blood circulation and brain metabolism, has a vasodilating, antihypoxic, anti-aggregation effect. It inhibits phosphodiesterase and increases the content of cyclic adenosine monophosphate in cells, which leads to a decrease in the content of calcium in the cytoplasm of smooth muscle cells and relaxation of myofibrils.
Dilates the blood vessels of the brain, increases blood flow, mainly in ischemic areas, improves the supply of oxygen to the brain.
Promotes the utilization of glucose and increases the level of catecholamines in the central nervous system, stimulates the metabolism of norepinephrine and serotonin in brain tissues. Reduces platelet aggregation, blood viscosity, increases the deformability of red blood cells and normalizes venous outflow against the background of reduced resistance of brain vessels.
Systemic blood pressure decreases slightly. In the elderly and senile age, the sensitivity of brain vessels to the relaxing effect of Vinpocetine increases, which is due to the sensitization of the adenylate cyclase system – cyclic adenosine monophosphate during aging.
Indications
Acute and chronic insufficiency of cerebral circulation. Dyscirculatory encephalopathy, accompanied by memory disorders, dizziness, headache.
Post-traumatic encephalopathy. Vascular diseases of the retina and choroid of the eye.
Hearing impairment of vascular or toxic origin, Meniere’s disease, dizziness of labyrinth origin. Vegetative-vascular dystonia in menopausal syndrome.
Contraindications
- acute phase of hemorrhagic stroke, severe coronary heart disease, severe arrhythmias, hypersensitivity to any of the components of the drug.
- pregnancy (possible placental bleeding and spontaneous abortions, probably as a result of increased placental blood supply).
- lactation period (when using the drug, it is necessary to stop breastfeeding).
- children under 18 years of age (due to insufficient data).
Side effects
From the cardiovascular system: Â ECG changes (ST depression, QT interval prolongation), tachycardia, extrasystole, blood pressure lability, hot flashes.
From the digestive system: Â dry mouth, nausea, heartburn.
From the central nervous system: Â sleep disorders (insomnia or increased drowsiness), dizziness, headache.
Other: Â allergic reactions, general weakness, increased sweating.
Interaction
When used concomitantly with warfarin, a slight decrease in the anticoagulant effect of warfarin has been reported.
With simultaneous parenteral use of vinpocetine and heparin, the risk of bleeding may increase.
How to take, course of use and dosage
Inside, after meals,3 times a day.
The initial daily dose is 15 mg.
The maximum daily dose is 30 mg.
The duration of treatment is up to 3 months.
For diseases of the kidneys or liver, the drug is prescribed in the usual dose.
Overdose
Currently, no cases of overdose of the drug Vinpocetine have been reported.
Treatment: Â gastric lavage, taking activated charcoal; if necessary, conduct symptomatic therapy.
Special instructions
The presence of prolonged QT syndrome and the use of drugs that cause prolongation of the QT interval requires periodic monitoring of the ECG.
In case of lactose intolerance, it should be noted that one tablet contains about 225 mg of lactose.
