Composition

Active ingredients:

Xantinol nicotinate – 150 mg;

water for injection up to 1 ml

Pharmacological action

Peripheral vasodilators. Purine derivatives.

Indications

• obliterating diseases of the vessels of the limbs: Raynaud’s disease, Buerger’s disease, occlusive disease, atherosclerosis;
• diabetic angiopathy vascular and retina;
• vascular thrombosis and embolism, pastabilities syndrome;
• venous leg ulcers, pressure ulcers, hard healing wounds;
• transient ischemic attacks, and other disorders of cerebral circulation;
• coronary atherosclerosis, hypercholesterolemia and hypertriglyceridemia;
• prenatal and postnatal asphyxia;
• migraine, Meniere’s disease.

Contraindications

• acute bleeding;
• hemorrhagic stroke;
• acute myocardial infarction;
• mitral stenosis;
• acute heart failure;
• chronic heart failure in the stage of decompensation;
• hypotension;
• gastric and duodenal ulcer in the acute phase;
• acute renal failure;
• glaucoma;
• pregnancy (especially the first trimester);
• breastfeeding;
• hypersensitivity to Xantinol nicotinate.

Side effects

• decreased blood pressure;
• transient feeling of warmth, redness of the skin, weakness, dizziness;
• rarely-nausea, diarrhea, anorexia, gastralgia;
• with prolonged use – impaired glucose tolerance, increased levels of transaminases and alkaline phosphatase, uric acid in the blood serum;
• allergic reactions.

Interaction

When used concomitantly with drugs that have a hypotensive effect (beta-blockers, ergot alkaloids, alpha-blockers, sympatholytics, ganglioblockers), with strophanthin, a sharp increase in their effect is possible.

With the simultaneous use of Xantinol nicotinate with theophylline drugs, an increase in the concentration of theophylline in blood plasma is observed and the risk of its toxic effect increases.

Anticonvulsants (carbamazepine, diphenine), barbituric drugs accelerate the metabolism of Xantinol nicotinate and reduce its plasma concentrations.

Combined oral contraceptives, macrolide antibiotics (clarithromycin, erythromycin), interferon drugs and antifungal agents (ketoconazole, fluconazole) change the severity of the effect of Xantinol nicotinate, slow its metabolism and increase the concentration of the drug in plasma.

When using Xantinol nicotinate in smokers, its plasma concentrations are lower than in non-smokers.

How to take, course of use and dosage

Apply intramuscularly and intravenously.

Adults are intramuscularly administered at a dose of 300-900 mg (2-6 ml of 15% solution) per day daily, for 2-3 weeks.

Intravenously, a slow jet is administered at a dose of 1-2 ml of a 15% solution 1-2 times/day, for 5-10 days (the patient should be in a horizontal position).

Intravenous drip is administered at a rate of 40-50 drops / min, the average dose is 150-900 mg. Dilute with a 15% solution up to 10 ml in a 5% dextrose solution from 200 to 500 ml or in a 0.9% sodium chloride solution up to 200 ml.

In acute disorders of blood supply to tissues, simultaneously with parenteral use of Xantinol nicotinate,300 mg 3 times a day is used orally.

Children are prescribed at the rate of 5-10 mg / kg / day.

Overdose

Symptoms of intoxication occur 4 to 5 hours after use of Xantinol nicotinate and continue for 12 hours.

At first, hot flashes, hyperemia of the upper half of the trunk, nausea, dizziness, tachycardia, decreased blood pressure are possible; later – fever, agitation, loss of consciousness, areflexia, tonic-clonic convulsions, vomiting.

There is no specific antidote. Treatment measures include drug withdrawal, maintenance and symptomatic therapy aimed at eliminating the disorders that have occurred, respiratory support, blood pressure monitoring, and relief of convulsive syndrome.

Special instructions

Use with caution in patients with labile blood pressure.

With extreme caution, it is used simultaneously with antihypertensive drugs.

Influence on the ability to drive motor vehicles and manage mechanisms.

Use with caution in drivers of vehicles, as well as in patients engaged in potentially dangerous activities.
