Pharmacological action
The Active ingredient of Xyzal-levocetirizine, an enantiomer of cetirizine, belongs to the group of competitive histamine antagonists. Levocetirizine has an effect on the histamine-dependent stage of allergic reactions, and also reduces the migration of eosinophils, reduces vascular permeability, and restricts the release of inflammatory mediators.
It prevents the development and eases the course of allergic reactions, has an anti-exudative, antipruritic effect, practically does not have an anticholinergic and antiserotonin effect. In therapeutic doses, it has practically no sedative effect.
Indications
For adults and children over 6 years of age: treatment of symptoms of year-round and seasonal allergic rhinitis and allergic conjunctivitis, such as conjunctival hyperemia, itching, sneezing, lacrimation, hay fever (pollinosis); urticaria, including chronic idiopathic urticaria, Quincke’s edema; other allergic dermatoses accompanied by itching and rashes.
Contraindications
Hypersensitivity to any of the components of the drug or piperazine derivatives.
Severe form of chronic renal failure (creatinine clearance less than 10 ml / min). Children’s age (up to 6 years). With caution-chronic renal failure (correction of the dosage regimen is required), elderly age (glomerular filtration rate may decrease).
Side effects
Headache, fatigue, drowsiness, dry mouth.
Rarely-migraine, dizziness, dyspeptic disorders, allergic reactions (angioedema, rash, urticaria, pruritus).
Interaction
Concomitant use with macrolides or ketoconazole did not cause significant ECG changes. Co-use with theophylline (400 mg/day) reduces the total clearance of levocetirizine by 16% (theophylline kinetics does not change).
When used in therapeutic doses, no data on interaction with alcohol have been obtained. However, it is necessary to refrain from drinking alcoholic beverages.
How to take, course of use and dosage
It is applied orally with food or on an empty stomach, washed down with a small amount of water, without chewing. Adults and children over 6 years of age: the daily dose is 5 mg (1 tablet).
Elderly patients (assuming normal renal function) do not need to reduce the dose.
For patients with chronic renal failure, the dose is reduced twice (1 tablet every other day) with creatinine clearance from 30 to 49 ml / min and 3 times (1 tablet every 3 days) with creatinine clearance from 10 to 29 ml / min.
Patients with hepatic insufficiency do not need to adjust the dosage regimen. The duration of admission depends on the disease. For the treatment of pollinosis, it is prescribed on average for 1-6 weeks. For chronic diseases (year-round rhinitis, atonic dermatitis), the duration of treatment can increase to 18 months.
Overdose
Symptoms: Â it may be accompanied by signs of intoxication in the form of drowsiness, in children, an overdose of the drug may be accompanied by anxiety and increased irritability. Treatment: Â if symptoms of overdose occur (especially in children), the drug should be discontinued, gastric lavage, activated charcoal intake, and symptomatic therapy should be performed. There is no specific antidote.
Special instructions
An objective assessment of the ability to drive a car and work with mechanisms did not reliably reveal any adverse events when prescribing the recommended dose of 5 mg, however, it is advisable to refrain from engaging in potentially dangerous activities that require increased concentration of attention and speed of psychomotor reactions.
