Adults and children over 12 years of age – 10 mg of cetirizine (1 tablet,20 drops or 2 scoops of syrup).
Children from 6 to 12 years – 10 mg of cetirizine (1 tablet,20 drops or 2 measuring spoons of syrup) 1 time a day or 5 mg of cetirizine (10 drops or 1 measuring spoon of syrup) 2 times a day, morning and evening.
Children from 2 to 6 years – 5 mg of cetirizine (1/2 tablet,10 drops or 1 measuring spoon of syrup) 1 time a day or 2.5 mg of cetirizine (5 drops or 1/2 measuring spoon of syrup) 2 times a day, morning and evening.
Children from 1 to 2 years – 2.5 mg (5 drops) 2 times a day.
With caution:Â chronic renal failure of moderate and severe severity (dosage adjustment is required), elderly age (glomerular filtration rate may decrease).

Pharmacodynamics
Cetirizine belongs to the group of competitive histamine antagonists, blocksH1-histamine receptors, and has virtually no anticholinergic and antiserotonin effects. It has a pronounced anti-allergic effect, prevents the development and facilitates the course of allergic reactions. It has antipruritic and antiexudative effects.
It affects the early stage of allergic reactions, and also reduces the migration of inflammatory cells; inhibits the release of mediators involved in a late allergic reaction. Reduces capillary permeability, prevents the development of tissue edema, relieves smooth muscle spasm. Eliminates the skin reaction to the introduction of histamine specific allergens, as well as to cooling (with cold urticaria). In therapeutic doses, it has practically no sedative effect. Against the background of course admission, tolerance does not develop.
The effect of the drug begins after 20 minutes( in 50% of patients), after 1 hour (in 95% of patients) and persists for 24 hours.
Suction. After oral use, cetirizine is rapidly and well absorbed from the gastrointestinal tract. Tmax — approximately 30-60 minutes.
Food intake does not significantly affect the amount of absorption, but in this case the rate of absorption is slightly reduced.
Distribution. Cetirizine binds to plasma proteins by approximately 93%. The value ofVd islow (0.5 l / kg), the drug does not penetrate the cell. The drug does not penetrate the BBB. Penetrates into breast milk.
Metabolism. Cetirizine is poorly metabolized in the liver to form an inactive metabolite. With 10-day use at a dose of 10 mg, accumulation of the drug is not observed.
Output. Approximately 70% — through the kidneys, mostly unchanged.
The systemic clearance is approximately 54 ml/min.
After a single dose of a single dose, the T1/2 value is about 10 hours. In children aged 2 to 12 years, the T1/2 value decreases to 5-6 hours. Hemodialysis is ineffective.
Pharmacokinetics in special patient groups
Patients with impaired renal function. With impaired renal function (creatinine clearance below 11-31 ml / min) and in patients undergoing hemodialysis (creatinine clearance below 7 ml / min), the T1/2 value increases 3-fold, Cl decreases by 70%.
Elderly patients and patients with chronic diseases. Against the background of chronic diseases and elderly patients, there is an increase in T1/2 by 50% and a decrease in Cl by 40%.
Optional for film-coated tablets
Patients with chronic liver diseases (hepatocellular, cholestatic and biliary cirrhosis). In patients with cirrhosis of the liver, there is an increase in T1/2 by 50% and a decrease in total clearance by 40% (dosage adjustment is required only with a concomitant decrease in glomerular filtration rate).
Contraindicated: pregnancy and lactation.

From the digestive system: Â dry mouth, dyspepsia.
From the central nervous system: Â headache, drowsiness, fatigue, dizziness, agitation, migraines.
Allergic reactions: Â skin rash, angioedema, urticaria, pruritus.
No clinically significant interaction of cetirizine with other drugs has been established. Co-use with theophylline (400 mg / day) leads to a decrease in the total clearance of cetirizine (theophylline kinetics does not change). At the recommended doses, it does not enhance the effect of ethanol (when its concentration in the blood is not more than 0.8 g / l), however, it is recommended to refrain from taking ethanol during treatment with the drug.

Symptoms: Â possible drowsiness, lethargy, weakness, fatigue, headache, tachycardia, increased irritability, urinary retention, dry mouth, constipation (most often when taking 50 mg of cetirizine per day).
Treatment: Â symptomatic therapy. Gastric lavage, use of activated charcoal. No specific antidote was identified. Hemodialysis is ineffective.
Patients with impaired liver and/or kidney function and the elderly should consult a doctor before taking the drug (see “Pharmacokinetics”, “Dosage and use”).
Influence on the ability to drive a car or perform work that requires an increased rate of physical and mental reactions. During the treatment period, it is necessary to refrain from engaging in potentially dangerous activities that require increased concentration of attention and speed of psychomotor reactions.
Simultaneous use of drugs that depress the central nervous system and alcohol is not recommended.
