Composition

Ceftriaxone (in the form of sodium salt) 1 g
Pharmacological action
Pharmaceutical Group: antibiotic-cephalosporin. Pharmacology: Cephalosporin broad-spectrum antibiotic of the third generation for parenteral use.
Bactericidal activity is caused by suppression of bacterial cell wall synthesis. It is resistant to the action of most beta-lactamases of gram-negative and gram-positive microorganisms.
Active against the following microorganisms: gram – positive aerobes-Staphylococcus aureus (including penicillinase-producing strains), Staphylococcus epidermidis, Streptococcus pneumoniae, Streptococcus pyogenes, Streptococcus spp. of the viridans group; gram-negative aerobes: Acinetobacter calcoaceticus, Borrelia burgdorferi, Enterobacter aerogenes, Enterobacter cloacae, Escherichia coli, Haemophilus influenzae (including penicillinase-forming strains), Haemophilus parainfluenzae, Klebsiella spp. (including Klebsiella pneumoniae), Moraxella catarrhalis, (including penicillin-producing strains), Morganella morganii, Neisseria gonorrhoeae (including penicillinase-forming strains), Neisseria meningitidis, Proteus mirabilis, Proteus vulgaris, Serratia spp. (including Serratia marcescens); individual strains of Pseudomonas aeruginosa are also sensitive; anaerobes: Bacteroides fragilis), Clostridium spp. (except Clostridium difficile), Peptostreptococcus spp.
It has in vitro activity against most strains of the following microorganisms, although the clinical significance of this is unknown: Citrobacter diversus, Citrobacter freundii, Providencia spp. (including Providencia rettgeri), Salmonella spp., including Salmonella typhi, Shigella spp. ; Streptococcus agalactiae, Bacteroides bivius, Bacteroides melaninogenicus. Methicillin-resistant staphylococci are also resistant to cephalosporins, including ceftriaxone, and many strains of Group D streptococci and enterococci, including Enterococcus faecalis, are also resistant to ceftriaxone.
Indications
Bacterial infections caused by sensitive microorganisms: infections of the abdominal cavity (peritonitis, inflammatory diseases of the gastrointestinal tract, biliary tract, including cholangitis, empyema of the gallbladder), pelvic infections, lower respiratory tract infections (including pneumonia, lung abscess, pleural empyema), acute otitis media, infections of bones and joints, skin and soft tissues (including infected wounds and burns), urinary tract infections (complicated and uncomplicated), uncomplicated gonorrhea, bacterial meningitis, bacterial septicemia, Lyme disease.
Prevention of postoperative infections.
Infectious diseases in immunocompromised individuals.
Contraindications
Hypersensitivity (including to other cephalosporins, penicillins, carbapenems), hyperbilirubinemia in newborns, newborns who are indicated for intravenous use of solutions containing Ca2+.
With caution.
Premature infants, renal and / or hepatic insufficiency, ulcerative colitis, enteritis or colitis associated with the use of antibacterial drugs, pregnancy, lactation.
Side effects
From the digestive system: nausea, vomiting, diarrhea, transient increase in the activity of hepatic transaminases, cholestatic jaundice, hepatitis, pseudomembranous colitis.
Allergic reactions: skin rash, pruritus, eosinophilia; rarely-angioedema.
From the hematopoietic system: with prolonged use in high doses, changes in the picture of peripheral blood (leukopenia, neutropenia, thrombocytopenia, hemolytic anemia) are possible.
From the side of the blood coagulation system: hypoprothrombinemia.
From the urinary system: interstitial nephritis.
Effects due to chemotherapeutic action: candidiasis.
Local reactions: phlebitis (with intravenous use), soreness at the injection site (with intravenous use).
Interaction
Ceftriaxone, suppressing the intestinal flora, interferes with the synthesis of vitamin K.
Therefore, when used concomitantly with drugs that reduce platelet aggregation (NSAIDs, salicylates, sulfinpyrazone), the risk of bleeding increases.
For the same reason, when used simultaneously with anticoagulants, there is an increase in the anticoagulant effect.
When used concomitantly with loop diuretics, the risk of developing nephrotoxic effects increases.
How to take, course of use and dosage
Individual approach. Enter IV or iv 1-2 g every 24 hours or 0.5-1 g every 12 hours. Depending on the etiology of the disease, an intravenous dose of 250 mg can be used once.
The daily dose for newborns is 20-50 mg / kg; for children aged 2 months to 12 years-20-100 mg / kg; the frequency of use is 1 time/day. The duration of the course is determined individually.
In patients with impaired renal function, dosage adjustment is required to take into account the values of creatinine clearance.
Maximum daily doses: for adults-4 g, for children-2 g.
Overdose
Treatment: symptomatic.
Hemodialysis and peritoneal dialysis are not effective.
Special instructions
Patients with hypersensitivity to penicillins may have allergic reactions to cephalosporin antibiotics.
Use with caution in patients with severe renal impairment.
Ceftriaxone solutions should not be mixed or administered simultaneously with other antimicrobials or solutions.
In newborns with hyperbilirubinemia, especially in premature infants, it can be used under strict medical supervision.
