Pharmacological properties

Pharmacodynamics .

The pharmacological action is due to anthraglycosides (mainly sennosides A and B), as well as other glycosides. When applied orally, senna glycosides pass through the stomach and small intestine unchanged, without being absorbed.

In the large intestine, anthraglycosides are broken down by intestinal bacteria into pharmacologically active anthrons and anthranols, which irritate the colon’s interoreceptors, increase peristalsis, and accelerate bowel emptying.

Anthrones and anthranols prevent the absorption of water and electrolytes, promote the diffusion of water into the intestinal lumen. Due to the osmotic effect, the fecal masses are loosened and their volume increases, which contributes to the laxative effect of anthraglycosides.

The laxative effect manifests itself in 8-10 hours.

Pharmacokinetics.

Metabolites of senna anthraglycosides are practically not absorbed, mainly excreted in the faeces, as well as in the urine. They are also released with sweat, penetrate into breast milk.

Indications

-Chronic constipation, hypo-and atony of the intestine;

– regulation of stool in hemorrhoids, proctitis, anal fissures;

– cleansing the intestines before diagnostic and therapeutic procedures.

Use during pregnancy and lactation

The drug should not be taken during pregnancy (efficacy and safety have not been sufficiently studied). During treatment with the drug, breast-feeding is recommended to stop, because the components of the drug can penetrate into breast milk and cause frequent loose stools in the baby.

Contraindications

• Increased individual sensitivity to drugs Senna or other components of the drug;

• uterine bleeding, intestinal obstruction, spastic constipation, acute inflammatory bowel disease (Crohn’s disease, ulcerative colitis, appendicitis), peritonitis, peptic ulcer of the stomach and duodenum, gastrointestinal bleeding, strangulated hernia, cystitis, pain in the abdomen of unknown origin;

• severe violations of water-electrolyte balance;

• pregnancy, lactation;

• children’s age up to 12 years.

With caution

It is used with caution in diseases of the liver and / or kidneys, in conditions after oral surgery, in children from 12 years of age.

Side effects

From the gastrointestinal tract: rarely-spastic abdominal pain, nausea, vomiting and diarrhea, flatulence, atony of the large intestine (with prolonged use), pigmentation of the large intestine (deposition of melanin pigment in the mucous membrane of the large intestine).

Allergic reactions:  rarely-pruritus, erythema, skin rash.

From the genitourinary system:  yellowish-brown or reddish staining of urine (due to metabolites excreted by the kidneys; this is not a pathological sign and passes after discontinuation of the drug), proteinuria, hematuria (with prolonged use, especially in high doses).

Metabolic disorders:  hypovolemia, hypokalemia (with prolonged use, especially in high doses, against the background of taking diuretics, corticosteroids and cardiac glycosides), secondary hyperaldosteronism.

Other services:  fatigue, confusion, muscle cramps (with prolonged use).

Interaction

With prolonged use or use in high doses, the drug enhances the effect of cardiac glycosides and antiarrhythmic drugs due to a decrease in the potassium content in the body, disrupts the absorption of tetracyclines.

When used concomitantly with thiazide diuretics, glucocorticoids, licorice root preparations, the risk of hypokalemia increases.

Reduces the effectiveness of drugs that are slowly absorbed in the gastrointestinal tract.

How to take, course of use and dosage

Take orally 1 time a day in the evening before going to bed, washed down with water.

Adults and children over 12 years of age: 1 tablet at a time. If there is no effect, the dose can be increased to 2-3 tablets.

Use the lowest dose required to produce soft feces. When selecting the dose, the same dose should be used for several days, if there is no effect, gradually increase the dose by ½ tablet. If defecation does not occur during the maximum dose within three days, you should consult a doctor.

The maximum daily dose of the drug is 3 tablets. Do not use the drug for more than 2 weeks. If you need a course of use of the drug on the recommendation of a doctor, after two weeks of use, you should take a break for two weeks.

Overdose

Symptoms:  abdominal pain, diarrhea, vomiting, followed by loss of water and electrolytes.

Treatment:  symptomatic, replenishment of fluids and electrolytes (especially potassium) to restore electrolyte balance, which is especially important for children and the elderly.

Special instructions

The effective dose of the drug is determined individually. Treatment begins with the lowest possible dose (1 tablet). The development of diarrhea is a sign of an overdose of the drug, and in this case its dose should be reduced.

If you have a frequent or daily need for laxatives, you should consult a doctor to determine the cause of constipation. When constipation develops, it is recommended to eat foods rich in plant fibers, consume large amounts of fluids, and exercise.

Long-term use of the drug can lead to the formation of addiction with the need for a constant increase in its doses and a violation of the water-electrolyte balance (in particular, hypokalemia), as well as intestinal atony. In this regard, it is recommended to alternate taking the drug with other laxatives.

Pigmentation of the large intestine may be due to chronic use of laxatives containing hydroxyanthracene glycosides. This condition is reversible and passes after the withdrawal of such drugs.

After taking the drug, the urine may turn yellow-brown or reddish in color. This is not a pathological sign and passes after the drug is discontinued on its own.

Influence on the ability to drive vehicles and mechanisms

The use of the drug does not affect the ability to perform potentially dangerous activities that require increased concentration of attention and speed of psychomotor reactions (driving vehicles, working with moving mechanisms).
