Active ingredient:

Pharmacological action
Normalizing the rheological properties of blood, improving microcirculation, vasodilating.
Indications
Violations of the peripheral circulation of atherosclerotic (for example intermittent claudication);
diabetic angiopathy;
trophic disorders (e. g. leg ulcers, gangrene);
cerebrovascular disease (consequences of cerebral atherosclerosis, as, for example, impaired concentration, dizziness, memory loss);
ischemic and post-stroke conditions of circulatory disorders in the retina and choroid;
otosclerosis;
degenerative changes in the background of the pathology of the vessels of the inner ear and hearing loss.
Contraindications
Hypersensitivity to the components of the drug;
massive bleeding;
extensive retinal hemorrhages;
brain hemorrhages;
acute myocardial infarction;
severe arrhythmias;
severe atherosclerotic lesions of the coronary or cerebral arteries;
uncontrolled arterial hypotension;
age up to 18 years;
pregnancy and lactation (breastfeeding);
hypersensitivity to other methylxanthines.
Caution should be used in patients with:
hypotension (risk of reducing blood pressure), congestive heart failure, impaired renal function – KK less than 30 ml/min (risk of accumulation and an increased risk of side effects), with severely impaired liver function (risk of accumulation and an increased risk of side effects), increased tendency to bleeding, including the use of anticoagulants or when violations of the blood coagulation system (risk of more severe bleeding), after the recent surgery.
Side effects
Nervous system disorders: headache, dizziness, anxiety, sleep disorders, seizures.
From the skin and subcutaneous fat: hyperemia of the skin of the face, flushes of blood to the face and upper chest, edema, increased fragility of nails.
From the digestive system: xerostomia, anorexia, intestinal atony, feeling of pressure and overflow in the stomach, nausea, vomiting, diarrhea.
From the cardiovascular system: tachycardia, arrhythmia, cardialgia, angina progression, decreased blood pressure.
From the hemostatic system and hematopoietic organs: leukopenia, thrombocytopenia; pancytopenia, bleeding from the vessels of the skin, mucous membranes, stomach, intestines, hypofibrinogenemia.
From the sensory organs: visual impairment, scotoma.
Allergic reactions: pruritus, hyperemia of the skin, urticaria, angioedema, anaphylactic shock.
Interaction
When used together, Trental potentiates the action of a number of antihypertensive and hypoglycemic agents (both insulin and oral hypoglycemic drugs).
Simultaneous use of pentoxifylline and theophylline can lead to an increase in theophylline levels in the blood and increase its side effects.
How to take, course of use and dosage
Inside, by swallowing whole, during or immediately after a meal, with a sufficient amount of water. The dosage is set by the doctor in accordance with the individual characteristics of the patient.
The usual dose is 1 tablet of Trental ® 400 2 or 3 times a day. The maximum daily dose is 1200 mg. In patients with impaired renal function (creatinine clearance below 30 ml/min), the dosage may be reduced to 1-2 tablets per day. Dose reduction, taking into account individual tolerability, is necessary in patients with severe hepatic impairment.
Treatment can be initiated in small doses in patients with low blood pressure, as well as in people who are at risk due to a possible decrease in blood pressure (patients with severe CHD or hemodynamically significant cerebral vascular stenosis). In these cases, the dose can only be increased gradually.
Overdose
Symptoms: dizziness, vomiting, drop in blood pressure, tachycardia, arrhythmia, redness of the skin, loss of consciousness, chills, areflexia, tonic-clonic convulsions.
If you develop the symptoms mentioned above, you should immediately consult a doctor. Treatment: symptomatic. Special attention should be paid to maintaining blood pressure and respiratory function.
Convulsive seizures are removed by the introduction of diazepam. When the first signs of overdose appear (increased sweating, nausea, cyanosis), immediately stop taking the drug.
Provide a lower position of the head and upper torso. Monitor the free patency of the respiratory tract.
Special instructions
Careful monitoring is necessary in patients with severe cardiac arrhythmias (risk of worsening arrhythmia), hypotension (risk of further lowering blood pressure), impaired renal function with creatinine clearance less than 30 ml/min (risk of accumulation and increased risk of side effects), severe liver function disorders (risk of accumulation and increased risk of side effects) and an increased tendency to bleeding, for example, when using anticoagulants or with disorders in the blood coagulation system (risk of more severe bleeding).
Caution should be exercised when prescribing Trental to patients with a history of gastric and duodenal ulcers; patients who have recently undergone surgery (the risk of bleeding is increased, and therefore systematic monitoring of hemoglobin and hematocrit levels is necessary).
In patients with hypotension and labile circulation, parenteral use of Trental is prescribed gradually, since it is possible to reduce blood pressure (up to collapse), and in some cases – angina attacks. Patients with heart failure should receive appropriate treatment to compensate for blood flow. At the same time, the introduction of a large volume of liquids should be avoided.
In patients with impaired renal function, the dosage regimen should be selected individually.
