Pharmacological action

Pharmaceutical group:

Pharmaceutical action:

 Vasonite is a derivative of xanthine. Improves microcirculation in areas of impaired blood supply. Improves the rheological properties of blood (fluidity) by affecting the pathologically altered deformability of red blood cells, increases the elasticity of red blood cell membranes, inhibits the aggregation of red blood cells and platelets, and reduces the increased blood viscosity.

The mechanism of action of the drug is associated with the inhibition of phosphodiesterase and the accumulation of cAMP in vascular smooth muscle cells and in blood cells. Pentoxifylline inhibits the aggregation of platelets and red blood cells, reduces the level of fibrinogen in blood plasma and increases fibrinolysis, which leads to a decrease in blood viscosity and an improvement in its rheological properties.

Improves the supply of oxygen to tissues in areas of impaired blood circulation (including in the extremities, central nervous system, and to a lesser extent – in the kidneys). With occlusive damage to the peripheral arteries (intermittent claudication), it leads to an increase in walking distance, elimination of night cramps, calf muscles, and reduction of pain at rest.

When the cerebral circulation is disturbed, it improves symptoms. It has a weak myotropic vasodilating effect, slightly reduces OPSS and slightly dilates the coronary vessels.

Indications

Circulatory disorders in patients with:  atherosclerosis, diabetes, Raynaud’s disease;

treatment of trophic ulcers, treatment of cerebral circulation disorders.

Use during pregnancy and lactation

Application of the drug Vasonitis during pregnancy is contraindicated.

If it is necessary to use the drug during lactation, stop breastfeeding.

Contraindications

• acute myocardial infarction
• heavy bleeding
• acute hemorrhagic stroke
• chronic heart failure.

Side effects

From the digestive system:  possibly – nausea, vomiting, diarrhea, dry mouth, decreased appetite, feeling of fullness in the abdomen, epigastric pain, increased activity of liver enzymes (ALT, AST, LDH) and alkaline phosphatase, cholecystitis, hepatitis, jaundice; rarely-unpleasant taste in the mouth, hypersalivation.

From the central nervous system:  relatively rare – headache, dizziness, restlessness, sleep disturbances, seizures.

From the side of the senses:  rarely-conjunctivitis, scotoma, blurred vision, ear pain.

From the cardiovascular system:  when using the drug in high doses, it is possible to reduce blood pressure, tachycardia, angina pectoris, cardiac arrhythmias, hyperemia of the facial skin, ” flushes ” of blood to the skin of the face and upper chest, edema.

Allergic reactions:  possibly-pruritus, skin rash, urticaria, angioedema; rarely-anaphylactic shock.

Hematopoietic and hemostatic disorders:  rarely-bleeding (from the vessels of the stomach, intestines, skin and mucous membranes), thrombocytopenia, leukopenia, pancytopenia, hypofibrinogenemia, changes in the picture of peripheral blood, aplastic anemia. In this regard, it is necessary to conduct regular monitoring of the blood picture.

Other services:  rarely – malaise, increased fragility of nails, changes in body weight, nasal congestion.

Interaction

Pentoxifylline can enhance the effect of drugs that affect the blood coagulation system (indirect and direct anticoagulants, thrombolytics), anitibiotics (including cephalosporins-cefamandol, cefaperazone, cefotetane), valproic acid.

Increases the effectiveness of antihypertensive drugs, insulin and oral hypoglycemic drugs. Cimetidine increases the concentration of pentoxifylline in plasma (risk of side effects).

Co-use with other xanthines may lead to excessive nervous excitement in patients.

How to take, course of use and dosage

Tablets covered with an enteric coating are swallowed whole, washed down with a small amount of water.

The daily dose is divided into 3 doses. The initial dose is 600 mg / day.

As the condition improves, the dose can be reduced to 300 mg / day.

Prolonged dosage forms are prescribed 2-3 times a day.

Overdose

Symptoms:  nausea, dizziness, decreased blood pressure, increased body temperature (chills), tachycardia, drowsiness or agitation, loss of consciousness, areflexia, possible tonic-clonic convulsions, vomiting like “coffee grounds” (as a sign of gastrointestinal bleeding), arrhythmias.

Treatment:  gastric lavage, then carry out symptomatic therapy. There is no specific antidote. In case of vomiting with traces of blood, gastric lavage is unacceptable.

Special instructions

Treatment should be carried out under the control of blood pressure. In patients with diabetes mellitus taking hypoglycemic drugs, use in large doses may cause severe hypoglycemia (dose adjustment is required).

When prescribed simultaneously with anticoagulants, it is necessary to carefully monitor the parameters of the blood coagulation system. In patients who have recently undergone surgery, systematic monitoring of Hb and hematocrit is necessary.

The administered dose should be reduced in patients with low and unstable blood pressure. In the elderly, it may be necessary to reduce the dose (increase bioavailability and decrease the rate of elimination).

The safety and efficacy of pentoxifylline in children are poorly understood. Tobacco smoke can reduce the therapeutic effectiveness of the drug. Compatibility of the pentoxifylline solution with the infusion solution should be checked on a case-by-case basis.
